- Clauses RFQ See attached RFQ. Please follow the instruction on the Spreadsheet and submit timely. Thank you. In accordance with the terms of FAR 12.603: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number for this requirement is RFQ 3904193 (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84. (iv) This is full an open under NAICS code 541519 (v) The contract line items and quantities can be found below or in the attached documentation. All option periods/quantities are included. (vi) There is no statement of work. (vii) Period of performance is 9/21/2015-9/20/2016 with 4 12-month options. (viii) The provision at 52.212-1, Instructions to Offerors—Commercial, applies to this acquisition and a statement regarding any addenda to the provision. (ix) This is a brand name only requirement, and will be evaluated as the lowest-priced, technically acceptable offeror. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications—Commercial Items, with its offer, or submit a printout to their Online Reps and Certs. (xi) The clause at 52.212-4, Contract Terms and Conditions—Commercial Items, applies to this acquisition. The clause is attached with Addenda (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders [MB([2] —Commercial Items, applies to this acquisition. (xiii) N/A (xiv) There is no Defense Priorities and Allocations System (DPAS) rating assigned to this acquisition. (xv) Submit offers to the CO, through email, by noon on Friday, August 18 th.
