Description

This is a combined synopsis/solicitation for commercial services prepared in accordance with FAR parts 12 and 13. This announcement constitutes the only solicitation. Solicitation number VA248-15-Q-2051 is issued as a Request for Quote (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-82, effective 25 August 2014. This solicitation starts 09/10/2015 and will end 09/23/2015 at 2:00 pm EST. Network Contracting Office (NCO) 8 is seeking potential qualified Contractors that meet Joint Commission Standards to provide Durable Medical Equipment (DME) on behalf of the Miami VA Healthcare System. This solicitation will require the contractor to store and deliver assigned durable medical equipment to include supplies, materials, equipment, transportation of equipment, equipment services, labor, supervision, patient education, safety management, infection control, as necessary for veterans. Occasionally, the contractor may be required to store excess equipment through adequate warehouse space as per the requested amount of the supported facility. The purpose of this contract will be to provide DME services 24 hours per day, 365 days a year. Miami's area of responsibility covers all Department of Veterans Affairs Beneficiaries residences in designated counties (Broward, Dade and Monroe). The intent for this contract is to meet or exceed the requirements for a one year contract period. This solicitation is set-aside for service-disabled veteran owned small business and Veteran Owned Small Business Contractors (SDVOSB), and Veteran Owned Small Business (VOSB) Contractors. The North American Industry Classification System (NAICS) for this procurement is 621610, Home Health Care Services; size standard is 500 employees. This will be a Firm Fixed Price, Indefinite Delivery Indefinite Quantity Contract for a period of one (1) year. The government anticipates awarding a single award contract. All qualified SDVOSB and VOSB sources are encouraged to submit a Quote. All offerors must be verified in the Vendor Information Pages, https://www.vip.vetbiz.gov/Default.aspx and registered in the System for Award Management (SAM) database, see https://www.sam.gov and complete the online Reps and Certs. Amendments to the solicitation will be posted to this website only. All offerors must obtain amendments at this website; hard copies will not be mailed. All questions must be submitted in writing via email by September 16th at 11 AM EST. The contractor must ensure that we receive the proposal on time and NO late quotes will be accepted.
