- Applicable Clauses and Provisions Pricing Schedule Environmental Management System Flyer Past Performance Questionnaire Performance Work Statement Combined Synopsis/Solicitation (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FA5613-15-R-0026, and the solicitation is issued as a request for proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-71. (iv) This is not a set-aside. The North American Industry Classification System (NAICS) code for this solicitation is 811310. (v) The 700 CONS/LGCA Flight has a requirement to purchase Maintenance, Safety Inspections and Repair of Hoists in the Kaiserslautern Military Community (KMC), for the period 01 April 2016 through 30 September 2016, with options to extend the performance through 30 September 2020. Offerors shall use Attachment 1, Pricing Schedule, which includes all contract line item numbers, quantities and units of issue, to fill in all unit prices and total amount blocks, for the base period and all option periods. Award will be made as Firm Fixed Price Contract and Hours Per Call is based on Historical Workload Averages (vi) Attachment 2, Performance Work Statement (PWS), contains the d escription of the requirement for Maintenance, Safety Inspections and Repair of Hoists in the Kaiserslautern Military Community (KMC). (vii) Places of performance and acceptance are identified in Appendix B of the PWS, contained in Attachment 2. All offerors must propose FOB (free on board) destination. Required period of performance: Base Period: 01 April 2016 - 30 September 2016 1 st Option Period: 01 October 2016 - 30 September 2017 2 nd Option Period: 01 October 2017 - 30 September 2018 3 rd Option Period: 01 October 2018 - 30 September 2019 4 th Option Period: 01 October 2019 - 30 September 2020 (viii) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jul 2013), applies to this acquisition. Addenda to this provision are included in Attachment 3, Applicable Clauses and Provisions. (ix) The provision at FAR 52.212-2, Evaluation - Commercial Items (Jan 1999), applies to this acquisition. Specific evaluation criteria contained in paragraph (a) of that provision are included in Attachment 3, Applicable Clauses and Provisions. (x) Offerors shall i nclude a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Nov 2013), Alternate I (Apr 2011), with their offer, or complete paragraph (b) of 52.212-3, if the offeror has completed the annual representations and certifications electronically at http://www.sam.gov. (xv) Offers are due 09 October 2015 not later than 14:00 hrs (2:00 pm) Central European Time (CET). Further details can be found in the addendum to 52.212-1 - Instruction to Offerors, in attachment 3 (xvi) A pre-proposal conference and site visit will be held on 24 September 2015, at 9:00 (a.m) in accordance with AFFARS 5352.215-9001, Notice of Pre-Proposal Conference (May 1996), and FAR 52.237-1, Site Visit (APR 1984), as stipulated in Attachment 3, Applicable Clauses and Provisions, pages 1 and 28. Offerors shall provide the names of the attendees not later than 18 September 2015 to the points of contact identified in the provision to allow sufficient time for approval of base entry.
