Description

SOLIC N68096-15-T-1026 US Naval Hospital Guam is Soliciting a Firm-Fixed-Price standing order type contract for the supply and delivery of laboratory consumables. Period of performance is 01 October 2015 through 30 September 2016. NAICS: 325412. Procurement is conducted in accordance with FAR 12 and 13.5. This requirement is a Small Business set-aside. Multiple delivery schedules are required per line item. Brand Name Only. There are refrigerated/perishable, and shelf-life requirements on line items. Door-to-door shipping direct to Agana Heights, Guam 96910. FOB: Destination. Preservation and packaging shall afford adequate protection against damage, deterioration and corrosion during shipment from supply source. All reagents shall be packaged, crated or consolidated as a single order for overseas delivery. Reagents shall be compliant with Food and Drug Administration requirements to market and deliver a medical product for use in the U.S., even should delivery be requested outside the United States. Responsible sources must be active in the System for Award Management (SAM) and a Registered-User in the Wide-Area-Work-Flow (WAWF) Electronic Invoicing Program. Include DUNS number, Cage Code, and Tax ID Number. Phone calls are not accepted.
