Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-15-Q-0421 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. The associated NAICS code is 339112 and size standard is 500 employees. This is a full and open competition requirement. The requirement is for Phaco Unit and accessories. Delivery location is the Louis Stokes Cleveland VA Medical Center (VAMC), 10701 East Boulevard, Cleveland, OH 44106-1702 and shall be FOB destination. Delivery required within 30 days of contract award. Network Contracting Office (NCO) Columbus, OH requires the following items, brand name or equal: Item Number 1 - StellarisPC with Laser, item number BL14304 - StellarisPC, Combined System with integrated 532nm laser, includes 2 lamp modules, wireless integrated foot pedal - 1 EA. Item Number 2 - Stellaris Ultrasound Phaco Handpiece, item number BL3170 - 2 EA. Item Number 3 - Stellaris Fragmentation Handpiece, item number BL3270 - 2 EA. Item Number 4 - Adapter, Alternate Tray, item number BL4364 - 1 EA. The vendor will provide any labor and equipment needed for proper assembly of the equipment. This includes any uncrating, assembling, and installation of components. Installation will also include testing the equipment. Testing shall include any tasks determined by the vendor to be necessary to ensure the machines are free of craftsmanship defects, all component parts are functioning within manufacturer's specification. The VA Biomedical Engineering staff will determine if testing results are satisfactory to begin internal acceptance and fit for use. The vendor will provide one year warranty on Phaco unit, as well as one year replacement warranty for the Handpieces and six months replacement warranty for Demo Handpieces. The Government will perform technical review on all quotes considered for this procurement and make a final determination if the quotes are technically acceptable. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-72. Offers will be evaluated on the lowest price technically acceptable. Offers should also include best expected delivery date.
