- Statement of Work Attachment 1 Statement of Work dated 8 September 2015 Sources Sought Synopsis for F3G4BA5201AW04, Vbeam Perfecta one-time preventative maintenance/repair. This is a Sources Sought Synopsis; there is no solicitation available. Requests for a solicitation will not receive a response. This Sources Sought Synopsis is published for market research purposes. This market research will be used to help determine if this may be a competitive acquisition, or if a set-aside for any small business program is appropriate (Small Business, 8(a), HUBZone, Service-Disabled Veteran Owned (SDVO) small businesses). Please review this announcement, including all attachments, in their entirety. Request that prospective offerors complete and return a Capabilities Statement that describes the comparable services and its specifications as described below in Attachment 1 (Statement of Work) to identify firms who possess the capability to provide Vbeam Perfecta one-time preventative maintenance/repair for the 99th MDG at the Mike O' Callaghan Medical Center at Nellis AFB, NV. The Government is in no way obligated to do business with or to enter into any form of contract with any person, firm or other entity that receives or responds to this announcement. The Base Operation Support Section is seeking potential sources, including small business sources. In addition to the information requested above, responding parties must also indicate their size status in relation to the applicable North American Industry Classification System (NAICS). The NAICS Code is 811219 with a Size Standard of $20.5M. Submit Capabilities Statements (e-mail preferred) by 3:30 p.m. PST on 11 September 2015. Send Capabilities Statements or other correspondence to James Eustace at james.eustace.2@us.af.mil and cc Karen Cusson at karen.cusson@us.af.mil. Be advised that all correspondence sent via e-mail shall contain a subject line that reads " F3G4BA5201AW04- Vbeam Perfecta one-time preventative maintenance/repair." Note that e-mail filters at Nellis AFB, NV are designed to filter e-mails without subject lines or with suspicious subject lines or contents.
