- QASP STATEMENT OF WORK Preventative Maintenance and Service Support Agreement for Roche Equipment - Light Cycler Instruments and MagNaPure Instrument in support of the United States Health Command (USAPHC). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 05-82. The combined synopsis/solicitation number is W91ZLK-15-T-0395 used as a request for proposal (RFP). The U.S. Army Contracting Command, Aberdeen Installation Contracting Division, Aberdeen Proving Ground, Maryland is soliciting this requirement as unrestricted. The requirement is under the associated North American Industry Classification System (NAICS) Code is 811219 and the Business Size Standard (in millions) is 20.50 Other Electronic & Precision Equipment Repair & Maintenance. The Government contemplates award of a Firm-Fixed Price type contract, Lowest Price Technically Acceptable (LPTA) under FAR Part 12, Acquisition of Commercial Items. Description of Requirement: This requirement is for Preventative Maintenance and Service Support Agreement for various Light Cycler Instruments and one (1) MagNaPure Instrument in support of the United States Health Command (USAPHC), Aberdeen Proving Ground, Maryland. The instruments are trademarks of Roche. Price proposal shall also be in accordance with the attached Statement of Work (SOW), Quality Assurance Plan (QASP, and combined synopsis solicitation. The intended period of performance is one year from the date of award for a twelve (12) month base period and two (2) twelve (12) month option periods. Acceptance and inspection shall be at Destination Technical Evaluation Criteria The service and maintenance to be conducted by the contractor are to include: - Unlimited Technical Phone Support - Loaner Instrument if the repair cannot be performed over the phone. - Free shipping for the loaner and serviced instruments - Warranty parts for equipment located at Aberdeen Proving Ground - Maryland and various regions. - Shipment of the government equipment for repair to Roche, and the return of the loaner when the government owned equipment repair is complete. All Offeror's shall fill-out cost of each job listed for a twelve (12) month base period and two (2) twelve (12) month option periods. Partial quotes will not be evaluated by the Government. Award will be made on the basis of the lowest evaluated price proposal meeting or exceeding the acceptability standards for non-cost factors in accordance with FAR 15.101-2. 52.212-2, Evaluation Commercial Items (Oct 2014) The contract will be awarded to the Lowest Price Technically Acceptable (LPTA) offeror. The technical evaluation will be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.
