Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. The solicitation number is VA258-15-R-0297 and is being issued as request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83 (Effective 07/02/2015). This acquisition is UNRESTRICTED and the associated NAICS code is 623220- Residential Mental Health and Substance Abuse Facilities and size standard is 15 Million. The Government anticipates awarding a Firm-Fixed Priced Indefinite Delivery Indefinite Quantity, type contract. This Request For Proposal (RFP) closes on September 14, 2015 at 12:00 PM EST. Proposals must be received on or before the closing date and time of this RFP. Submit all written proposals via email to Christina.Tayman@VA.GOV. Follow guidance as spelled out in 52.212-2 Evaluation-Commercial Items (OCT 2014). All responding vendors must be registered in SAM https://www.sam.gov/portal/public/SAM in order to receive an award. The Government is seeking a contractor to provide services as part of its Community Based Health Care for Homeless Veterans (HCHV) program. This will provide transitional housing for severely mentally ill (SMI) veterans for the Phoenix VA Health Care System located in Phoenix, AZ. The services shall be provided directly to the veterans who are eligible beneficiaries specifically authorized by the Phoenix, VA Health Care System, 650 E. Indian School Road, Phoenix, AZ 85012. This Indefinite Delivery Indefinite Quantity (IDIQ) Contract is for a (1) One Year Base Period from October 01, 2015 to September 30, 2016.
