- Blood Culture, Platelet Culture, and AFB Culture Bottles The proposed contract action is for commercial items which the Naval Medical Center San Diego intends to solicit on a sole source basis under the authority of FAR 13.106-1 (b) (1) (i). This proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source, Biomerieux, Inc. located at 100 Rodolphe St., Durham, NC 27712-9402. Only Biomerieux's product line is compatible with the BacT/Alert 3 instrument that the Government already owned. All other offerors may submit a quote that will be reviewed by the Government. However, a determination by the Government not to compete with this proposed contract based upon responses to this solicitation is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement in the future. This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in FAR subpart 12.6, using Simplified Acquisitions Procedures for commercial items found at FAR 13, as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-15-T-0319 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 325413: Size: 500. This is an unrestricted solicitation. The contractor shall provide blood culture, platelet culture, and AFB culture bottles that are FDA-cleared and compatible with the command-owned BacT/Alert 3D automated blood culture instrument in accordance with the Statement of Work (SOW). Supplies that require special handling, e.g., refrigeration, time sensitive, open immediately, etc., will be marked in a clear manner with a label that is easily read, understandable and larger than 3" x 3". The label will be placed in three or more locations on the container (top & sides.) Unit of Issue: Box; Price: _______________; Extended Price: _______________ Naval Medical Center San Diego, California 92134 Blood Culture Testing/BacTAlert STATEMENT OF WORK 1.0 INTRODUCTION 1.1 Organization: The Naval Medical Center San Diego (NMCSD) is responsible for providing the U.S. Navy, other military branches, and their military families with reliable quality health care. In addition to maintaining status as one of the nation's premier care providers, NMCSD is responsible for preparing for deployable support of operational forces, as well as shaping the future of military medicine through education, training and research. 1.2 Background and Objective: In order to provide the expected standard of care for patients NMCSD must be capable of providing certain in-house laboratory tests that are ordered on a routine basis. Blood culture is a critical test used for the diagnosis of serious infections and requires standardized equipment to ensure uniform incubation conditions and frequently reading of culture bottles to detect growth as expeditiously as possible. The contractor will provide equipment and services in accordance with the requirements as stated herein. 2.0 REQUIREMENT 2.1 Task Description and Deliverables: All equipment and accessories, testing methodologies, and supplies must be approved by, and compliant with the U.S. Food and Drug Administration (FDA). Additionally,the equipment and supplies shall be in compliance with OSHA requirements. Training, Technical Support and Install The Contractor must furnish all necessary equipment, supplies, parts, tools, and labor to perform this contract to include the appropriate number of incubators to meet the blood culture volume at NMCSD, computer and software database, and all services associated with keeping the equipment in good working order • The Contractor must act in accordance with commercial practices. • The Contractor must include Medical Liability clauses for non-personal medical services. • All sub-contracted referral facilities used by the Contractor shall also meet the above requirements. • The Contractor must call the originator regarding clarification of work submitted or questions about specific requirements. Unusual terminology and non-standard test requests must be verified with the originator when unclear. The contractor will provide trained technical service representatives to install all instrumentation. The Contractor must provide instructions and materials necessary to instruct NMCSD personnel in the use of the Contractor's analyzer systems, e.g. operation of, maintenance on and troubling shooting techniques for the analyzers. The Contractor will make all consumable materials necessary for the performance of various types of blood and platelet cultures available through Cardinal Prime Vendor to allow ease of ordering and distribution of these supplies throughout NMCSD and clinics. A service plan must be included in the contract for the base year as well as all option years. This service plan will include at a minimum all of the following: 24 hours a day/7 days per week telephone technical support with on-site support dispatched as necessary, provide hardware and software upgrades when available, and repairs and periodic preventative maintenance at no cost. The contractor is hereby advised that the United States Government and agencies are exempted from state and local government tax by virtue of article 6 of the United States Constitution. No exemption certificate is required. 2.2 Task Personnel Requirements Technical and Personal Skills: All technical staff and service engineers employed by the contractor must have backgrounds in engineering, science, or laboratory technologies. The contractor must provide a rigorous training and credentialing program used to assess competency of and proficiency for those skills necessary to successfully trouble shoot and fix equipment before being assigned to an account to provide such services. 3.0 Classification - Unclassified Privacy Act Statement: "Pursuant to Title 5 United States Code 552a(m)(1), the contractor and all employees of the contractor working under this contract are required to comply with the requirements of 5 U.S.C. 552a ("The Privacy Act of 1974"). 4.0 Period of Performance - Base year with four option years with base year date to begin once contract is awarded. Proposed base year is 1 October 2015 - 30 September 2016. 5.0 Personal Services NON PERSONAL SERVICES STATEMENT: Contractor employees performing services on the equipment under this order will be controlled, directed, and supervised at all times by management personnel of the contractor. Contractor management will ensure that employees properly comply with the performance work standards outlined in the statement of work. Contractor employees will perform their duties independent of, and without the supervision of, any Government official or other Defense Contractor. The tasks, duties, and responsibilities set forth in the task order may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal policy, obligating the appropriated funds of the United States Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee, or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR).
