This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (1) The solicitation number VA259-15-Q-0619 is issued as a request for quotation (RFQ). (2) This solicitation is under NAICS code 339112. (3) Contract Line Item number:1 (4) Description of requirements for the items to be acquired: Line Item Product#DescriptionQty 1TCM400-4Transcutaneous O2 Monitoring1 System with four O2 modules Includes TMC400 base unit, (4) O2 modules, (2) blind modules, (4) E5250 tcp02 electrodes (1) box Each : D282 Fixation Kit, D826 Accessory Kit, Users handbook and power cord Item Descriptions and Specifications Transcutaneous O2 Monitoring System Scope The essential requirement is for the contractor to provide Brand Name or Equal to the Radiometer Transcutaneous O2 Monitoring System with four O2 Modules. If quoting an Equal product must provide a side-by-side features comparison. This will be a firm fixed-Price contract. Background Billings VA Clinic requires the TCM400-4 Transcutaneous O2 Monitoring System in order to provide care to Veteran patients at the clinic. System should include TMC400 base unit, (4) O2 modules, (2) blind modules, (4) E5250 tcp02 electrodes, (1) box each D282 Fixation Kit, D826 Accessory Kit, users handbook and power cord. (5) Delivery: 30 Days ARO, FOB Destination, Deliver To: VA Billings Majestic Attn: Surgical Services 1776 Majestic Lane Billings, MT 59102 (6) Clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (7) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Product shall meet or exceed the specifications listed in the line item. Any substitution must provide detailed product descriptions and justification as to how the product deemed met or exceeded. No partial quote or shipment will be accepted. (8) Offeror shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (9) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (10) Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition with consideration to 52.219-6, Buy American Act-Supplies.
