This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. Solicitation number VA263-15-Q-0866 is issued as a request for quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. The purpose of this contract is to procure services for collecting and analyzing water samples for Legionella in accordance with VHA Directive 1061, "PREVENTION OF HEALTHCARE-ASSOCIATED LEGIONELLA DISEASE AND SCALD INJURY FROM POTABLE WATER DISTRIBUTION SYSTEMS" and for collecting and analyzing water samples to verify compliance with "Infection Control Standards for VA Dental Clinics," 21 October 2013, and EPA Safe Drinking Water Act regulations in Dental Offices within the Nebraska-Western Iowa VA Health Care System, 4101 Woolworth Avenue, Omaha, NE 68105-1850. Additional requirements are listed within the attached solicitation documents. This procurement is being solicited as a Total Small Business set aside. Prospective offerors must be registered and current in the System for Award Management (SAM) database prior to contract award: (https://www.sam.gov/portal/public/SAM/). Quotes will only be accepted from responsible contractors. The NAICS code for this solicitation is 541380 and the small business size standard is $15 million. A list of the contract line items, quantity, and unit of measure are listed within the attached solicitation document(s) as well as the provisions and clauses that apply to this acquisition. General instructions to Offerors for Quote Submission are also included in the attached solicitation document(s). Quotes are due to the Contracting Office by Tuesday, September 15, 2015 at 4:00 PM local time. Quotes are required to be emailed to jason.williamson@va.gov prior to the solicitation closing date and time. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Officer before the solicitation closing.
