Description
Synopsis: Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. Network Contracting Office (NCO) 3 of the Department of Veterans Affairs is currently conducting a market survey for qualified firms capable of providing "SERVICE MAINTENANCE CONTRACT FOR ANESTHESIA MACHINE LEO1616" for the New Jersey VA Healthcare System. New Jersey VA Health Care System is requesting full service contract covers for maintenance, repairs, parts, software upgrade and technical support on government owned "Anesthesia Machine" from within the healthcare system. The North American Industry Classification Systems (NAICS) is 811219. Responses to this notice must be submitting in writing (email or fax) and must be received not later than September 1, 2015 at 10:00AM. Email: Janice.Brooks@va.gov and Fax: 718-741-4722. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, a capability statement, examples of same or similar work performed at other facilities, DUNS number and address, point of contact and social-economic. Interested parties who do not submit the above information on or before the response due date and time will not be considered for this procurement. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 811219. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Statement of Work Background: The Biomedical Engineering department at VA New Jersey Health Care System is requesting full service contract covers for maintenance, repairs, parts, software upgrade and technical support on government owned "Anesthesia Machine" at the New Jersey Healthcare System, East Orange Campus. Fabius Tiro Serial #: ASEA-0197 Period of Performance: Base period: Date of Award - 12 Months Option year one:12 Months - 24 Months Option year two:24 Months - 36 Months Place of Performance/Place of Delivery: New Jersey Healthcare Systems, East Orange OR Department 385 Tremont Avenue East Orange, NJ 07018 Scope or Tasks (for services): The contractor shall provide all necessary labor, equipment, tools, material, parts, repair services, preventive maintenance, maintenance support and software upgrades on the Anesthesia Machine as required by the NJHCS. It is the intention that all equipment repairs and/or maintenance will be performed in accordance with manufactures' guidelines will result in the equipment operating according to manufacturers' specification and will be done in accordance with industry standard practices. In addition, the contractor shall also provide maintenance services which include troubleshooting, replacement of worn parts, calibration/adjustment, cleaning and lubricating in accordance with the OEM, remote desktop support and telephone support. -General Requirement: oRequest for technical telephone support will be responded within two (2) hours. oVendor shall be on site to begin repairs within eight (8) hours of initiation service call by the VA NJHCS. oRepairs shall be completed and unit returns fully to manufacturer specification within sixteen (16) hours of initiation service call. oMajor assemblies replaced will be guaranteed against defects and/or failure for a period of one (1) year from date of installation. All other components will be guaranteed for a period of ninety (90) days. oWork to be performed, which is not covered under this contract, must be pre-approved by the contract specialist/officer and a separate purchase order will be issued for the work to be performed. Failure to obtain prior authorization will result in Non-Payment of charges incurred. -Parts: oAll parts will be provided and replaced as necessary at no additional charge. oOnly new parts will be used routinely. oUsage of refurbished parts will be subjects to the approval of the Biomedical Engineer and/or designee. -Documentation: oThe service representative must report to Biomedical to sign in upon arrival and sign out upon leaving. oThe service representative is required to leave a service report at the time work is completed prior to leaving the premises with any available Biomedical Shop personnel obtaining a signature. oThe service report shall contain at least the following information: "A brief description of the purpose call "Work performed "Parts used "Time spent for labor and travel oIf the work is not completed prior to leaving the premises, a service report will be left, as per the above procedures, indicating the equipment status. oNo payments will be made if the above procedures are not in compliance. -Special Requirement: oThe contractor must supply themselves with their own maintenance/repair manuals along with any other diagnostics software required to properly maintain the equipment in question. oService representatives are required to supply the NJHCS with Material Safety Data Sheets (MSDS) on any chemicals that are brought into the NJHCS prior to usage. The MSDS are to be given to the Safety Officer. A set of MSDS's must also remain at the site where the chemical is used. oVendor must comply with usage of Personnel Protective Gear as defined in 29CFR 19.10. oVendor must comply with all VA policies and regulations. -Hours of Coverage: oAll work performed under this contract will be during the hours of 8:00AM to 4:30PM, Monday through Friday excluding weekend and federal holidays. oOvertime will only be paid for emergencies and pre-approved situations by the Biomedical Engineering and/or designee. oEmergency service will be provided at all times in accord with the above.
