- Request For Quotation (RFQ) under the solicitation number FA5000-15-T-0198 for pick-up and disposal of Regulated Medical Waste (RMW) and Pathological waste at the Joint Base Elmendorf-Richardson (JBER), Alaska hospital This synopsis/solicitation is issued as a Request For Quotation (RFQ) under the solicitation number FA5000-15-T-0198 for pick-up and disposal of Regulated Medical Waste (RMW) and Pathological waste at the Joint Base Elmendorf-Richardson (JBER), Alaska hospital. All questions concerning this solicitation must be received by 12:00 PM, 3 September 2015. Deadline for receipt of quotes is 9 September 2015, 4:30 PM. All times listed are Alaska Local Time. CONTRACTING OFFICE ADDRESS: Department of the Air Force (DAF), Pacific Air Forces (PACAF), 673d Contracting Squadron, 10480 Sijan Ave, JBER AK, 99506-2500. DESCRIPTION: This is a solicitation for a commercial product; prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items under the authority of FAR Subpart 13, Simplified Acquisition Procedures, as supplemented with additional information included in this notice. Quotations are being requested and a written solicitation document will not be issued. SERVICES REQUIRED: A general description of services is listed below; however, offerors should refer to the attached Performance Work Statement (PWS) for a complete description of services requested required. This is to establish a non-personal services contract to provide pick-up and disposal of Regulated Medical Waste (RMW) and Pathological waste at the Joint Base Elmendorf-Richardson (JBER) Hospital. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn, is responsible to the Government. The Contractor shall provide all management, personnel, equipment, supplies, transportation, tools, materials, supervision, and other items and non-personal services necessary to pick-up and dispose of Regulated Medical Waste (RMW) and Pathological waste at the Joint Base Elmendorf-Richardson (JBER) Hospital as defined in the Performance Work Statement (PWS) except for those items specified as Government or student furnished property and/or services. The Contractor shall perform to the standards as specified in the contract and PWS. This includes the planning, coordination, and surveillance of the activities necessary to ensure disciplined work performance and timely resources application to accomplish all taskings under the contract. The Contractor shall be responsible for maintaining communication with the Contracting Officer (CO) and the Quality Assurance Personnel (QAP) (if applicable), and to immediately notify both the CO and the QAP of any problems that would prevent timely performance of this contract. The Contractor is responsible for and required to implement, and maintain management control systems necessary to plan, organize, direct, and control all activities under this contract. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83 as of 3 August 2015, Defense Federal Acquisition Regulation Supplement Publication Notice 20150602, effective 2 June 2015 and Air Force Acquisition Circular 2014-0406 Baseline effective 6 April 2015. This acquisition is solicited as a 100% Small Business Set-Aside. The applicable (North American Industry Classification System) NAICS code is 562211, Hazardous Waste Treatment and Disposal, and the business size standard is $38,500,000.00. Prospective vendors must ensure they meet all requirements in 52.212-1, 52.212-2, 52.212-3 and addendums as failure to follow all instructions may result in a quote being deemed technically unacceptable, non-responsive, and potentially ineligible for award.
