- Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify homeless shelters/facilities in or around Altoona, PA to provide contracted emergency residential services (CERS) for homeless Veterans. Services target homeless Veterans who are living on the streets, being discharged from institutions, and Veterans who recently became homeless and are in need of safe living arrangements while seeking permanent housing solutions. General scope of the procurement: the services will be provided for authorized Veterans within the catchment area of the James E. Van Zandt VA Medical Center (VAMC). The CERS facility will be required to provide services such as room & board, meals, therapeutic counseling, case management, transportation, medication storage, etc. The Government intends to award a 5-year BPA to procure these services. General requirements: the CERS facility must be licensed to operate its business and be located within 40 miles of the James E. Van Zandt VAMC in Altoona, PA. The facility must also be capable of passing an initial inspection conducted by select personnel from the VAMC. NCO 4 is seeking responses from CERS facilities that are interested in this procurement and consider themselves to have the resources, capabilities needed to provide the CERS services. Please respond with your: 1.Facility name, location, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), and person of contact (including telephone number & email address). 2.Facility information regarding its capabilities to provide the CERS services. Responses must be submitted by 11:00 AM (EST) September 9, 2015.
