Description
STATEMENT OF WORK A.GENERAL INFORMATION 1.Title of Project: Atlanta VA Medical Center's Homeless Program Annual Stand Down - October 17, 2015 Location: 1701 Hardee Ave SW. Atlanta, GA. 30310 (Fort McPherson Campus) 2. Scope of Work: The contractor shall provide all equipment and resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The contractor shall deliver; setup, breakdown, and pick-up all tables, chairs, heaters, tents, trees, and other items, as listed in the deliverable schedule (found below) for the annual Homeless Stand Down on October 17, 2015. 3. Background: Majority of the events held at the Homeless Stand Down are held outdoors. The rental items in this SOW are necessary to ensure the appropriate set-up of equipment is available to service the Veterans that will attend the event. 4. Period of Performance: Base and Option year contract Base year: Saturday, October 17, 2015 Year 1: October, (Day TBD) Year 2: October, (Day TBD) Year 3: October, (Day TBD) Year 4: October, (Day TBD) 5. Location of Services Provides: Atlanta VA Medical Center - Ft. McPherson Campus 1701 Hardee Avenue, SW Atlanta, Georgia 30310 5. Type of Contract: This contract shall be a firm-fixed price contract. B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. 2. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. 3. Where a written milestone deliverable is required in draft form, the VA will complete their review of the draft deliverable within ___ calendar days from the date of receipt. The contractor shall have ___ calendar days to deliver the final deliverable from date of receipt of the Government's comments. D. Deliverable One: Deliver all equipment on October 16, 2015. Task Two: Breakdown and pick-up of all equipment. Deliverable Two: Pick-up of all equipment must occur on October 17, 2015. E. EVALUATED OPTIONAL TASKS AND ASSOCIATED DELIVERABLES - N/A F. SCHEDULE FOR DELIVERABLES All deliverables must be received on Friday, October 16, 2015 in preparation for the Stand-down event on Saturday October 17, 2015 (Base year) subsequent years TBD. G. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. H. REPORTING REQUIREMENTS - N/A I. TRAVEL N/A J. GOVERNMENT RESPONSIBILITIES The government shall be responsible for ensuring the appropriate locations for deliverables are secured and that the Contractor will have appropriate access to those locations. K. CONTRACTOR EXPERIENCE REQUIREMENTS Contractor shall have necessary experience with the set-up and break-down of all deliverables. L. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. M.CONTRACTOR PERSONNEL SECURITY REQUIREMENTS All equipment will be delivered and setup outdoors and security access to VA building facilities is not required. The contractor shall be required to comply with all security policies/requirements while on Government property. N.SCHEDULE OF DELIVERABLES All deliverables must be received on Saturday, October 17, 2015 O.SALIENT CHARACTERSITICS - See Description of Tasks and Associated Deliverables - Above All deliverables must be in good to excellent condition.
