- Tax liability form. Please review, sign, and return with quote. Statement of Work Brand Name Letter for equipment. FBO RFQ for CCTV Synopsis/Solicitation The solicitation number is F1SEAD5217AG01. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13 as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83, DFARs Change Notice (DPN) 20150826 and AFAC 2015-0406. This acquisition is a 100% small business set-aside under NAICS 334220 with a small business size standard of 750 employees. Site Visit: The Government will not reimburse any contractor for cost incurred with the preparation, finalizing or submittal of the contractors quote for this RFQ. The Government will host an optional site tour, available to all interested Contractors, to allow viewing of the Governments current environment, infrastructure, work location and gather any questions for answering in regards to contract objectives. The site tour will be hosted for one day only, tentatively scheduled 4 business days after this RFQ posting date. All travel and lodging costs are the responsibility of the Contractor. The Contractor is responsible for providing their own transportation to the site. Any questions/answers to questions asked during the tour will be provided to all who attend the tour and all who have provided their contact information for the 5 day question and answer period. Description: Air Force Test Center (AFTC) Contracting Directorate, Edwards Air Force Base, CA is seeking to purchase the following Firm Fixed Price supply: Brand Name (please see attached Brand Name letter), INDIGOVISION, LTD, NVR AS-4000. No partial support responses will be accepted. This acquisition shall provide all resources for the following: This acquisition is to provide, repair along with modifying, designing, and installing the INDIGOVISION, LTD, NVR AS-4000 upgraded Closed Circuit Television (CCTV) Surveillance system giving the ability of added security throughout the Child Development Center (CDC) at Edwards Air Force Base. The system shall be located in Building 6457 to include in addition to the existing system; 1 Resource Room, 1 Kitchen, 1 Room "5", 1 Front office, 1 Front Desk, and 1 Lobby. This will be a repair/replace of 2 cameras, an installation of 5 additional cameras, and an installation of 6 additional monitors to the existing system. Please submit the following information with each quote: Cage Code, DUNS number, Delivery Time, and Date offer expires. Offerors are required to submit with their quote enough information for the Government to evaluate the minimum requirements detailed in this synopsis. Only the Lowest Priced, Technically Acceptable (LPTA) offeror will be requested to provide the INDIGOVISION, LTD, NVR AS-4000. Award will be made to the LPTA offer that is most advantageous to the Government. Offerors are instructed to complete the provision DFARS 252.209-7992, as attached, IN FULL TEXT.
