- Statement of Work The Food and Drug Administration (FDA) intends to award a purchase order for the recruitment and coordination of minors in the participation of in-store purchases of tobacco products, evidencing collection, and reporting. The requirement will be awarded using Simplified Acquisition Procedures in accordance with FAR Part 13. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/bids/quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number is FDA-15-1155444. This solicitation is issued as a Request for Quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83, August 3, 2015. This is a firm fixed price requirement. This requirement will be awarded as a purchase order for a period of 12 months. The associated North American Industry Classification System (NAICS) Code is 541611, Administrative Management and General Management Consulting Services. The size standard is $15,000,000. This notice is for Small Businesses and only Small Businesses (HUBZone, 8(a), Small Disadvantaged Businesses (SDB), Veteran-owned, Service-Disabled Veteran- owned (SDVOSB), and Woman-owned (WOSB)) shall submit offers. (v) Technical Specification of Requirement: See attached Statement of Work (SOW). (vi) The provision at FAR 52.212-1 Instructions to Offerors - Commercial Items (April 2014) applies to this solicitation. The following addenda have been attached to this provision: See attached SOW. (vii) The provision at FAR 52.212-2 Evaluation - Commercial Items (Oct 2014) applies to this solicitation. The following addenda have been attached to the clause: See attached SOW. (viii) Offerors must include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications- Commercial Items (Mar 2015) with its offer. (ix) The clause at FAR 52.212-4 Contract Terms and Conditions - Commercial Items (May 2015 ), applies to this solicitation. The following addenda have been attached to the clause: See attached SOW. (x) The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes of Executive Orders - Commercial Items (May 2015 ), applies to this acquisition. The following FAR clause also applies to this solicitation: FAR 52.217-8 Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor any time prior to the expiration of the contract. (End of Clause) (xi) The Defense Priorities and Allocations System and assigned rating are not applicable to this solicitation notice. (xii) An Offeror must be registered in the System for Award Management (SAM) before an award can be made to the Offeror. If the Offeror is not registered in SAM, the Offeror must go to the SAM website at http://www.sam.gov. (xv) QUOTES PROVIDED MUST ADDRESS ALL REQUIREMENTS.
