- W-9 MTC Self-Certification Form MTC Terms & Conditions DBA Wage Determination CBP Hello Everyone, This is a subcontracting opportunity with Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center. Tongue Point Job Corps Center is requesting proposals for hardwood floor refinishing. Details of the solicitation are in the attached request for quote (CBP# TPJCC Qtrs. K Hardwood Floor Refinishing 15-18.pdf.) Please accept this as your invitation to attend our pre-bid meeting for the project listed above at TPJCC, located at 37573 Old Highway 30., Astoria, OR 97103. The meeting will be conducted on Thursday, September 10, 2015 at 2:00pm (PST), with bids due on Wednesday September 16, 2015 at 5:00pm (PST). Please see the attached Construction Bid Package (CBP) for details. Please pay close attention to page 2 of the Construction Bid Package Instructions to Bidder. In addition to the bid sheet, all valid bids must include 3 (three) references, DBA Worker Classification/Rates, a W-9, and completed MTC vendor self-certification form (attached). Bidders must self-certify under the NAICS code listed in the CBP. (NAICS codes can be found at http://www.census.gov/eos/www/naics/) This solicitation is listed under NAICS code: 238330 Flooring Contractors. Note: A site-inspection will be required by the bidder or their appointed company representative for the bid to be considered valid. The best opportunity for this inspection will be during the pre-bid meeting at the time listed above.
