This requirement is to be delivered to the Charleston, SC CMOP, 3725 Rivers Ave, Suite 2 North Charleston, SC 29405-7038 NLT 4 September 2015. The NAICS Code for the requested Med-Surg item is 339112. THIS IS AN URGENT REQUEST. Description of required Med-Surg Procurement: "Description of request: DEXCOM G4 GLUCOSE SENSOR (Brand Name) "NDC: 008627004104 (NO SUBSTITUTES) "Item #: STS-GL-041 (NO SUBSTITUTES) "Quantity requested: 400 boxes (4 sensors per box = 28 day supply) "MUST CONTAIN THE FOLLOWING FEATURES: G4 PLATINUM SENOR; invasive (e.g. subcutaneous), disposable, for use with interstitial continuous glucose monitoring system up to 7 days "Supplier/Manufacturer: DEXCOM, INC. All other contract details including full specifications will be included in the Request for Quotes, which will be released on or about August 25, 2015.
