Attachment D - POSHO guidlines Attachment C - Project 3 - SOW SDH gate vehicle Attachment B - Project 2 - SOW SDH compound housing Attachment A - Project 1 - SOW Front gate consular visitor Combined Synopsis SF-1449 COMBINED SYNOPSIS/SOLICITATION RFQ SKU200-15-Q-0020 DESCRIPTION This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures found at FAR Subpart 13.5 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation on FedBizOpps; quotes are being requested and a written solicitation will not be issued. This is not to be construed as a commitment by the Government. All information is to be submitted at no cost to the Government NOTICE TO OFFEROR(S)/SUPPLIER(S) The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs This solicitation is set-aside 100% for small business. The following North American Industry Classification System (NAICS) and small business size standard applies: NAICS code314910 - Textile Bag and Canvas Mills Small business size standard in number of employees 500 The place of performance for this RFQ is the American Embassy Kuwait Solicitation SKU200-15-Q-0020 is issued as a Request for Quotation to obtain services from a responsible contractor to install the following three (3) awning projects at American Embassy in the State of Kuwait, as per attachment A, B, and C (Scope of Work), and attachment D (POSHO Office - Contractor's General Responsibilities for Construction Projects). -Project # 1: Front gate consular visitors awning -Project # 2: SDH compound housing awning -Project # 3: SDH gate vehicle awning Interested offerors are requested to provide a separate price to each project listed above. The Government reserves the right to award all three (3) projects or either number of these projects, subject to availability of funding A site visit is scheduled on September 2, 2015 at 10:00am Kuwait local time for prospective offerors to inspect the site/location. Offerors should contact/send id information details (maximum 2 persons per company) by email to the following POCs before August 30, 2015 to make appropriate access arrangements: 1.Zeyad Qishawi - Procurement Supervisor Tel: 2259-1234, Fax: 2259-1938 email: QishawiZI@state.gov 2.Norbert Dsouza - Purchasing Agent Tel: 2259-1215, Fax: 2259-1938 - email: DsouzaHN@state.gov Question Submission: Interested offerors must submit any questions concerning this solicitation by email to the above mentioned POCs no later than September 7, 2015 to enable the Embassy to respond. Questions and answers will be posted in FedBizOpps. Proposal Submissions: Quotes are due by September 14, 2015 at 14:00 Kuwait local time. All quotes must be clearly marked with the RFQ Number SKU200-15-Q-0020, and include a separate price per each project listed. Contractors are responsible for verifying receipt of their quotes to the designated location before the specified due date and time. Email is the only method of quote submission. Quotes, together with the signed amendment(s), if applicable, must be submitted electronically to both POCs mentioned above. FAR 52.212-1 INSTRUCTIONS TO OFFERORS -- COMMERCIAL ITEMS (APR 2014), is incorporated by reference ADDENDUM TO 52.212-1 Interested offerors are requested to submit the following Volume(s) with their proposal/quote: (1)VOLUME I - INTRODUCTION -Contractor Name, Commercial and Government Entity (CAGE) Code, and Dun & Bradstreet (DUNs) Number. -Vendors must be registered in the System for Award Management (SAM) to be eligible for award -Name, telephone numbers and e-mail address of person(s) to be contacted for clarification or questions to the quote. -Provision 52.212-3 Offeror Representations and Certifications -Commercial Items applies to this solicitation. The contractor shall either complete the on-line Offeror Representations and Certifications or return a completed copy of the Offeror Representations and Certifications with their quotation; Quotes received without the completed copy of the provision at FAR 52.212-3 or completed SAM representation may be removed from consideration. (2)VOLUME II - TECHNICAL -As stated, this solicitation has three (3) awning projects. Interested offerors are requested to provide Schedule/Timeline for each project -Requirements as per SOW (attachment A, B, and C) and attachment D (POSHO Office - Contractor's General Responsibilities for Construction Projects) -Experience/Past performance - Provide a list of contracts previously performed and points of contact with phone numbers After award, the Contractor has 2 calendar days to submit to the Contracting Officer a list of workers and supervisors assigned to this project for the Government to conduct all necessary security checks. It is anticipated that security checks will take 3-5 days to perform. For each individual the list shall include: Full Name Place and Date of Birth Current Address Civil Identification number The Contractor shall furnish below information to conduct all necessary security checks. -Vehicle make, model and registration number -List of all heavy duty tools/equipment required to be used for the project. -Copies of Passport, and Civil ID for all individuals who will perform this project. Individuals who are security cleared shall be allowed to enter and work on this project. Failure to provide any of the above information may be considered grounds for rejection and/or resubmittal of the application. Once the Government has completed the security screening and approved the applicants a badge will be provided to the individual for access to the site. This badge may be revoked at any time due to the falsification of data, or misconduct on site. (3) VOLUME III - PRICE The Contractor shall complete all work, including furnishing all labor, material, equipment, insurance, and services required under this purchase order for the following firm fixed price and within the time specified. This price shall include all labor, materials, all insurances, overhead and profit. Description Price Project # 1: Front gate consular visitors Project # 1: SDH compound housing Project # 1: SDH gate vehicle Total amount -To be considered timely. E-mail quote must be received in its entirety in the designated E-mail inbox by the due date and time for final quote submission. An E-mail quote that resides on a Government server, but has not appeared in the designated E-mail inbox by the due date and time for quote submission will be considered late in accordance with FAR 15.208(b). Therefore, it is incumbent upon the Quoter to ensure that its entire quote is received by the Agency by the designated due date. (End of Provision) 52.212-2 -- Evaluation -- Commercial Items (Oct 2014) The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Factor 1: Technical Capability -Provide a brief description of how you plan to perform the work listed in the performance work statement -Provide Schedule/Timeline for each project -Provide sample of awning fabric and knitting thread -Provide sketch/drawing of awning layout -Experience/Past performance - Provide a list of contracts previously performed and points of contact with phone numbers Factor 2: Price - Prices will be evaluated for reasonableness. Award will be made to the quoter submitting the lowest priced technically acceptable quote. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
