Description

THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS. This notice is issued for the purpose of collecting information only. The Department of Veterans Affairs, Network Contracting Office (NCO) 21, in Martinez, CA, is conducting a MARKET SURVEY to obtain information regarding (1) the availability and capability of qualified small business sources; (2) the socio-economic status of the small business source, such as Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) the size classification of the small business source, relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. ******* Small business concerns should be capable of furnishing home oxygen services, including providing oxygen equipment, Continuous Positive Air Pressure Devices (CPAP), nebulizers, ventilators, and other oxygen related supplies, as well as, providing in-home delivery, set-up, education, maintenance, repair and professional consultation on a 24/7/365 basis. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. States / Counties / Facilities Served: VA Central California Healthcare System serves the counties of - Fresno, Kings, Madera, Mariposa, Merced, Tulare, Kern, and San Luis Obispo California. The main VA facility is located in Fresno California. VA Palo Alto Healthcare System serves the counties of - Alameda, Calaveras, Monterey, San Benito, San Joaquin, San Mateo, Santa Clara, Santa Cruz, Stanislaus and Tuolumne, California. The main VA facility is located in Palo Alto California. San Francisco VA Medical Center serves the counties of - Del Norte, Humboldt, Lake, Marin, Mendocino, Napa, San Francisco, San Mateo and Sonoma, California. VA Northern California Healthcare System serves the counties of - Alameda, Amador, Butte, Colusa, Contra Costa, El Dorado, Glenn, Napa, Placer, Sacramento, Shasta, Siskiyou, Solano, Sutter, Tehama, Trinity, Yolo, and Yuba, California. The main VA facility is located in Martinez California. VA Pacific Islands Health Care System serves the entire state of -Hawaii. The main VA facility is located on the island of Oahu. VA Sierra Nevada Health Care System serves the following counties - -Nevada counties: Washoe, Humboldt, Pershing, Churchill, Mineral, Lyon, Carson City, Douglas, Storey, Esmeralda, Lander and Eureka; and California counties: Modoc, Lassen, Plumas, Sierra, Nevada, Placer, El Dorado, Alpine, and Mono. The NAICS code for this requirement is: 532291 POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1.Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), type of business, (i.e., small, small disadvantaged, woman-owned, Hub Zone, serviced-disabled veteran-owned small business, large business, etc.), under North American Industry Classification System (NAICS) 532291, which contains a size standard of $32.5M, and DUNS Number. Regarding the NAICS code, state under which NAICS code your company usually provides the requirements described in the attachment. Provide rationale for your answer if different from NAICS 532291. 2.Identify the geographic area(s) serviced by your company for home oxygen services 3.Provide a Statement of Capability that demonstrates the Offeror's past performance in providing Home Oxygen Services. Please include the following: a.staff expertise including their availability, experience, and formal and other training; b.current in-house capability and capacity to perform the work; c.prior completed projects of similar nature; d.corporate experience and management capability; and e.examples of prior completed Government contracts, references, and other related information. 4.Submit your response by no later than August 25th, 2015 to contract specialist Angela Oppenheimer, email angela.oppenheimer@va.gov. Submissions must be made via email only. AT THIS TIME, NO SOLICITATION EXISTS. DO NOT REQUEST A COPY OF THE SOLICITATION. A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE RELEASED. 5.Disclaimer and Important Notes: This notice does not obligate the Government to issue a solicitation or award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. 6.No Proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s.) 7.Responses shall be no longer than 2-3 pages maximum (one page is suggested).
