Description
Wage Determination 2005-2215(Rev.-18) Please complete this attached RFQ as part of your response. Other documentation shall be considered, but all information requested within the document is required. Performance Work Statement, dated 18 May 2015 In accordance with FAR 12.603(c)(2): This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation, which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-83. The Government intends to award a Firm-Fixed-Price Contract based on Lowest Price Technically Acceptable. The associated NAICS code for this acquisition is 811310 with a size standard of $7,500,000.00. The Request for Quote number is: FA4621-15-T-RJS2. McConnell AFB, KS 67221-3702 plans to acquire the following project: Crash Fire MX 0001 Inspections Inspections shall be performed IAW the attached SOW. Period of Performance: 1 October 2015 - 30 September 2016 3 Each $ $ 0002 Service Calls Service Calls shall be performed IAW the attached SOW. Period of Performance: 1 October 2015 - 30 September 2016 200 Hours $ $ 0003 Parts Draw Contractor shall be reimbursed for parts required for repair of the crash fire rescue trainer at cost IAW the attached SOW. Period of Performance: 1 October 2015 - 30 September 2016 1 Lot $6,000.00 NTE $6,000.00 NTE 1001 Inspections Inspections shall be performed IAW the attached SOW. Period of Performance: 1 October 2016 - 30 September 2017 3 Each $ $ 1002 Service Calls Service Calls shall be performed IAW the attached SOW. Period of Performance: 1 October 2018 - 30 September 2019 1 Lot $6,000.00 NTE $6,000.00 NTE In accordance with FAR 52.219-6, Notice of Total Small Business Set-Aside, this is a notice that this order is a set-aside for small business concerns. Only quotes submitted by small business concerns will be accepted by the Government. Any quote that is submitted by a contractor that is not a small business concern will not be considered for award. In accordance with FAR 52.232-18, Availability of Funds (Apr 1984), funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. Statement of Equivalent Rates for Federal Hires (May 2014) In compliance with the Service Contract Labor Standards statute and the regulations of the Secretary of Labor (29 CFR part 4), this clause identifies the classes of service employees expected to be employed under the contract and states the wages and fringe benefits payable to each if they were employed by the contracting agency subject to the provisions of 5 U.S.C. 5341 or 5332.
