- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W9114F-15-A-0009 is issued as a request for quotation (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50, effective 6 May 2011. Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 621991. The Regional Health Contracting Office-Europe has a requirement to establish a Blanket Purchase Agreement (BPA) for Human Skin Tissues and Corneal Tissues. The BPA prices shall include commercial door-to-door shipping to Landstuhl Regional Medical Center (LRMC), Germany. Shipping to the containerization site at New Cumberland is not authorized. (i) Quoters shall certify that all tissues offered shall be supplied by source facilities that are accredited by the American Association of Tissue Banks; (ii) Quoters shall certify that the source facilities are registered as a tissue establishment with the Food and Drug Administration (FDA); and (iii) Quoters shall certify that the source facilities are licensed by state agencies as required. Subfactor 2: CUSTOMS. Quoters shall submit a written narrative to show how it plans to clear all ordered items through the German customs, while ensuring package integrity and manufacturer-recommended transport temperature ranges are met and are acceptable. Subfactor 3: FDA APPROVAL. Quoters shall certify that all offered items (skin, bone and corneal tissues) shall be approved by the FDA. FACTOR II - PAST PERFORMANCE. Quoters shall submit past performance references stating the name, telephone number and e-mail address of present and past performance references. The Quoter's past performance information shall be for contracts of similar supplies/services and scope performed during the past five (5) years. The Government will evaluate Quoter's past performance information by making independent assessments of the past performance criteria evaluating the responses to the past performance questionnaires completed by the Quoters' references. The Government will also evaluate Quoter's past performance information that the Government independently obtains. FACTOR III: PRICE. Vendors shall provide their price quotes on Technical Exhibit #1. ADDENDUM TO FAR 52.212-2 The Government will award a Blanket Purchase Agreement (BPA) resulting from this solicitation to the responsible Quoter whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be made on the basis of the Lowest Price, Technically Acceptable (LPTA) quote. If any technical factor/sub-factor is rated Unacceptable, then the Government will no longer consider issuing a BPA to that Quoter, thus the Quoter will be ineligible for award. I. TECHNICAL EVALUATION STANDARDS. The Quoter's technical quote will be evaluated using the following standards for a final rating: ACCEPTABLE: A quote that meets the Government's technical requirements identified in the solicitation. UNACCEPTABLE: A quote that fails to meet the Government's technical requirements identified in the solicitation. II. PAST PERFORMANCE. The past performance evaluation will be evaluated based on the following two evaluation standards in making a rating of each Quoter's past performance questionnaire: ACCEPTABLE: Based on the Quoter's performance record, the Government has a reasonable expectation that the Quoter will successfully perform the required effort, or the Quoter performance record is Unknown. (See note below.) UNACCEPTABLE: Based on the Quoter's performance record, the Government has no reasonable expectation that the Quoter will be able to successfully perform the required effort. NOTE 1: In the case of a Quoter without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Quoter may not be evaluated favorably or unfavorably on past performance. Therefore, the Quoter shall be determined to have an Unknown past performance. In the context of Acceptability/Unacceptability, quote mark Unknown quote mark shall be considered quote mark Acceptable. quote mark III. PRICE. The Government will select the quote that is based upon the lowest evaluated price, technically-acceptable quote. Items on Technical Exhibit #1 were priced by the Quoter. SPECIAL NOTE: All Vendors must be registered in the System for Award Management (SAM) prior to award, and lack of registration shall make an offer ineligible for award.
