- Davis Bacon Wage Rates July 2015 Statement of Work Solicitation FA2835-15-R-0017 STATEMENT OF WORK For Remodel Area E Room 42 Location Building: 1614 By: Roland Hartz WORK ORDER NUMBER: 66044 DATED: 30 July 2014 •1. GENERAL INFORMATION: This project is intended to remodel area E Room 42 in accordance with all applicable National, Air Force, State and local building codes and regulations. This work will consist of supplying/furnishing all labor, materials, tools, equipment, shop drawings, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of Massachusetts Building Code and Massachusetts General Law requirements, state and federal codes, base standards and manufacturer's instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply. •2. SCOPE: Remodel Area E Room 42 ( high density insulation must be used in walls) •· Install STC45 rated walls •· Prior to the start of work coordinate work schedule with the Carol Bevis, Facility Manager and Tracy Romel, Physical Security Officer •· Remove ceiling tiles and ceiling grid in the conference room •· Remove carpet in the conference room •· Demolish and dispose of existing west and north wall within the conference room. In addition, demolish and dispose of wall in the hall from the existing door to the hallway. •· Install one (1) layer of 5/8" gypsum wallboard to the remaining two walls (east and south). The installation shall be per the specifications of a STC 45 rated wall. The wall shall be finished from floor to ceiling. •· Construct STC45 rated walls using ASTM International Classification E413 and E90 at the location of the two demolished walls. •· The two new walls shall have four (4) layers of 5/8" gypsum wallboard installed instead of the three (3) specified in Chapter 3 Fixed Facility SCIF Construction. •· Sound attenuation material shall be of a rigid material. •· Walls will extend from top of floor slab to the underside of floor/ceiling above. Where wall intersects other building elements, including but not limited to ductwork, floor joists, plumbing, electrical etc. gypsum board shall be fitted to maintain the STC rating of the wall. •· Seal all joints and penetrations with appropriate acoustical sealant. •· The contractor shall install all conduit within the walls (both new and existing) and pull all wires through the conduit for electrical, (data and communication by others) •· Prime and paint walls 1 coat primer and 2 finish color to match existing in area •· Install new carpet tiles and cove base (color TBD) •· Install new ceiling and grid in room •· Install Overly Vault door 3-0' x 7-0' with STC rating of 52/ with door closer and handle, install lock set supplied by customer •· Install wood trim around frame on both sides of door to match existing adjacent doors •· HAFB Environmental Contractor guide must be followed ** Door and lock must be installed by GSA certified locksmith, all workers must be US Citizens and provide proof of citizenship * * ** Hours of operation are 0630- 1830** •3. GENERAL REQUIREMENTS: Work includes, but is not limited to the following: •a. Provide resolutions of any technical problems that arise during the construction. •b. Should any change in scope be required, it must be coordinated with the Project POC and approved as a change order prior to performing the change in work. •c. Provide updates to Project POC. Any construction issues, changes to schedule, or schedule slippages with justification must be reported at the periodic updates. •d. The Contractor is solely responsible for compliance with the Occupational Safety and Health Act of 1974 (OSHA) and the protection of their employees and all sub-contractor employees. In performing work under this contract on a Government installation, the contractor shall report any accidents involving Air Force property/equipment damage, or contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the contracting officer within five days." •e. Provide three (3) copies of O & M manuals and warranty letter •f. Period of Performance shall be 120 day after contract award •4.
