- Brand Name Justification and Approval This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA4877-15-Q-0391 is issued as a request for quotation (RFQ). This solicitation document incorporates provisions and clauses of those in effect through Federal Acquisition Circular 2005-83, Defense Circular Number 20150803, and Air Force Acquisition Circular 2015-0406. This procurement is being issued as an unrestricted basis. The Associated North American Industrial Classification Standard (NAICS) code for this request is 238330 with a size standard $15,000,000.00 or under. The results of this requirement will be a firm fixed priced (FFP) contract. The right to make multiple or no award is reserved in the event it is advantageous to the Government to do so. Items called for by this RFQ have been identified in the schedule as brand name. The contractor shall provide the following Contract Line Item Number (CLIN): CLIN 0001- 1 (lot) 870 sq yrds Tandus Entwine broadloom carpet, 01603 Regal color, Tandus XL seam sealer, and Tandus B19 adhesive. 600 lin ft Johnsonite Traditional cover base, DC47 Brown color, and Mapie 575 cover base adhesive. Ardex Feather Finish used for all subfloor patching. Installation and removal of existing carpet to be included. THIS REQUIREMENT IS A BRAND NAME PER ATTACHED J&A. There will be a recommended site visit on Tuesday, 11 August 2015, 2:00 PM Arizona Local Time. All contractors attending the site visit will meet at the DMAFB visitors center at the Main Gate located at the intersection of Craycroft Road and Golf Links Road at 1:30 PM Arizona Local Time. To attend the site visit, please send the names of the people attending and whether driving private or company owned vehicles to Christopher Vilmont, christopher.vilmont@us.af.mil, and Jozlyn Brewton, jozlyn.brewton@us.af.mil, no later than 10 August, 2015 at 1:00 PM Arizona Local Time. It is highly encouraged for potential bidders to attend the site visit so any site conditions that may affect the bid can be identified. Shipping will be FOB Destination to The Mirage Club, 5555 E. Ironwood St., DMAFB, Tucson, AZ. 85707. The shipment should be marked for Lisa Chamberlain.
