- Attachment 2 Clauses and Provisions Orthotic Technician Attachment 1 Performance Work Statement Orthotic Technician JBKACC-15-PN-0002 RFQ Document This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is JBKACC-15-PN-0002 and is issued as a Request for Quotation (RFQ). This combined synopsis/solicitation incorporates provisions and clauses are those in effect through Federal Acquisition Circular 2015-83 and DFARS change notice 20150626. This purchase is restricted to HUBZone Small Business concerns. This NAICS code is 621111 and the size standard is $11,000,000.00 annual revenue. This is a requirement for Orthotic Technician Services (1 FTE) as described in this RFQ and the Performance Work Statement. Basis for award will be determined as Lowest Price Technically Acceptable (LPTA) based on the criteria listed in Clause 52.212-2 Evaluation - Commerical Items. This is a new requriement and does not have incumbents. The vendor shall submit the response to this RFQ via email to the Contract Administrator: Philip W. Napier, Ph: (210)671-0792, E-mail:philip.napier@us.af.mil no later than 10:00 a.m. CST on 17 August 2015- Tuesday (or sooner if possible). The Contracting Officer for this solicitation is Venetia C. Shaw; Ph: (210) 671-5906, E-mail: venetia.shaw.1@us.af.mil. Any quote, modification, revision, or withdrawal of a quote received after the exact time specified for RFQ is "late" and will not be considered, unless the Contracting Officer determines that accepting the late quote would not unduly delay the award and acquisition or it was the only quote received.
