- Wheelchair lift specifications for RFQ 2015-Q-17557FB Picture 5 Picture 4 Picture 3 Picture 2 Picture 1 Contract terms and Solicitation provisions for RFQ 2105-Q-17557FB This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this requirement is 2015-Q-17557 FB and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention (CDC) intends to award a firm-fixed-price purchase order to a contractor to install a wheelchair lift in accordance with the attached specifications. The lift will be installed at the National Institute for Occupational Safety and Health (NIOSH), Robert A. Taft Laboratories (Taft Building) at 1150 Tusculum Avenue, Cincinnati, OH, 45226. QUOTERS ARE ADVISED AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the quoter's best terms from a technical and price standpoint. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. THIS IS A TOTAL SMALL BUSINESS SET-ASIDE. The North American Classification System (NAICS) code for this procurement is 238290 with a small business size standard of $15 million. The Offeror must have an active registration with the System for Account Management (SAM) prior to award of the purchase order. in accordance with FAR Clause 52.204-7. Quotes are due on 08/13/2015, by 1:00 PM Eastern Time and must be sent to the attention of Florence Black via email at fpblack@cdc.gov. The Offeror must cite RFQ Number 2015-Q-17557 FB in the email subject line. Evaluation and award of the purchase order will be performed in accordance with FAR 13.106-2 Evaluation of Quotations or Offers and FAR 13.106-3 Award and Documentation. Special Notice: Any vendor wishing to see the area where the installation will occur is invited to attend a site visit scheduled for Friday, August 7, 2015 at 9:00 AM. Because access to CDC facilities is restricted, you must email your intent to attend the site visit no later than 10:00 AM, Thursday, August 6. The email must include the names and contact phone numbers for all persons who will attend. Access will be limited to US Citizens because of the lengthy period of time necessary to obtain clearances for non-US citizens. Send the email to fpblack@cdc.gov. After evaluation of all proposals, the government intends to award to the lowest priced technically acceptable offer.
