- This is a Notice of Sources Sought. This is not a request for a quote/proposal. ***No award will be issued with this notice***. Recreation Hall Repair/Refinish Louis Stoke VAMC Main Hospital Background: Sand/Scuff flooring in Recreation Hall. Apply new finishing product to floor. Scope: Provide all subfloor testing, field verifications, labor, material, equipment, supplies, and incidentals, etc. to remove existing finish on floor. Apply new finishing (Sherwin Williams DS 350VOC Poly or equal) product. Tasks: Project Phases: "This project will consist of three phases. "Each area will be a phase, with multiple days per phase (one week) required. The vendor must submit a proposed schedule for VA review/approval prior to start of work. (Approximately one third of the area will be completed per phase). "All work to be completed during day shift hours (7:00am - 5:00pm). Existing floor finish removal: "Install critical barriers. Use polyethylene sheeting from ceiling to floor around the affected area with a slit entry and covering flap. "Maintain the area under negative pressure with a HEPA filtered air/fan unit. "Block supply and return air vents within the containment area. "Strip the existing wood finish. New floor finishing: "Apply two coats of polyurethane finish product. (Sherwin-Williams DS 350VOC Poly or equal). This is an oil based clear coat product. "All furniture will be removed by VA before work begins. Place of Performance: Louis Stokes VA Medical Center of Cleveland 10701 East Blvd. Cleveland, Ohio 44106 1st Floor - Main Hospital The Government intends to Sole-Source this project to Fred & Son's, Inc. located at 27709 Joyce Dr. Columbia Station, Ohio 44028, if there no adequate competition is identified. This project is a Small Business Set-Aside using NAICS Code: 238330. The Small Business size standard is $15 million. Interested parties are required to submit a company brochure outlining their capabilities, and if not using the Sherwin Williams product provide a technical specification sheet for the polyurethane finish intended to be used. Interested parties must submit the company brochure and technical specification sheet (if required) to Matthew Curtis, at matthew.curtis1@va.gov NLT 3:00PM on 8/10/2015.
