- COMBINED SYNOPSIS / SOLICITATION DATE: 3 AUGUST 2015 PRODUCT: SOUND BOOTH FACILITY: Augusta VA Medical Center, Augusta, GA AGENCY: Department of Veterans Affairs SET-ASIDE: SERVICE DISABLED VETERAN OWNED SMALL BUSINESS (VIP) This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. Interested parties should contact the Contract Specialist, Joseph Locke, joseph.locke@va.gov for a copy of the solicitation so that interested offeror(s) may complete the SF1449 document. Issue Date: August 3, 2015 Closing Date: August 7, 2015, 5pm EST Only e-mailed requests received directly from the offeror are acceptable. A brief description of the requirement is as follows: Statement of Work Booth Installation for Downtown Vendor shall provide certified supervision and labor to provide for a turn-key replacement of an existing sound booth located at Charlie Norwood VAMC rooms 1D136, 1D137 and 1D139. The process shall be accomplished in two phases as described: Phase 1 - secure all booth support utilities to include electric circuits, fire suppression, and HVAC duct work and control/monitoring wiring. Systems shall be secured in compliance with applicable codes and in a manner in which provide for reconnecting to the new booth if required. Disassemble the existing booth, package in a manner suitable for transport and storage. Relocate the disassembled booth to a location identified by VA COR. Phase 2 - Provide and install new sound booth in location identified in Phase 1. Installation shall provide for a 100% functional sound boot which fully complies with all life safety and environmental functions to include conditioned air, electrical outlets, lighting and A/C controls. Vendor shall provide all required cosmetic work to include any patching, painting; decorative trim as to provide for a finished product which is suitable for a patient care environment. Performance: Vendor shall provide all certified supervision and technical labor to remove an existing sound booth and install Model 8069 by Medical Acoustics, Inc. Vendor shall provide and install the required A/C equipment, controls as to provide for a comfortable code compliant patient environment to include appropriate air exchanges. Vendor shall provide the VA with a test and balance report which verifies the installation meets code. Vendor shall provide and install a monitoring system for Metasys by Johnson Controls. Vendor shall provide for all site prep work as to provide for new booth installation to include installation of all required electrical circuits, fire sprinkler system, required HVAC connections to include VAV Boxes with controls and trim work. Vendor shall replace existing drop ceiling with new and perform all cosmetic work as to present a finished space suitable for a patient care environment. Vendor shall fully comply with established Infection Control requirements for a patient care environment; ensure the area in clean at the end of each work day. Upon completion of the installation, a function test shall be conducted by the vendor to ensure all systems and support systems perform as designed and to the satisfaction of the VA COR. Vendor shall provide the VA COR with a certificate of certification, stating booth meets all standards which govern sound booths. In addition vendor shall provide a document of completion to include test and balance report. Reporting: Vendor shall report to VA Representative Dr. Berry or designee at extension to attain required VA ID badges. Hours of Work: Work shall be accomplished in a manner as not to interfere with patient care and if necessary shall be performed over weekends. However all work shall be scheduled and agreed to by VA COR, Dr. Berry or designee. Normal working hours 8:00am - 4:30-pm. Level existing pit to accommodate new booth Provide electrical hookup to building. Possibly 3 new dedicated lines to breaker box needed. Material included. Ceritifed Electrician Provide HVAC hookup from booth to building (supply and return air). Existing HVAC boxes will need to be reconfigured in order to provide adequate air to the new suites. ETS to provide correct collars on silencers. Material included. Remove, dispose and replace drop ceiling. Includes material and labor. Replace trim along exterior of the booth to hide gap between booth and finished floor. Touch up drywall and paint where needed. Provide all floor and wall protection. Miscellaneous materials and labor. Remove 32' x 10' wall and 9' x 10' wall on top of booth and dispose offsite. Tie new drop ceiling into header and roof extensions. 12' Roof extenstions brovided by ETS. Forklift rental for 7-9 days with delivery & pickup. Negative air machine 2 weeks with materials to vent. Vendor shall provide a totally 100% functioning exam booth which meets all life safety, and environmental functions such as conditioned space and lighting.
