Hold Harmless Agreement Financial Responsibility Questionnaire Past Performance Questionnaire Past Performance References FA2823-15-R-4020 SOLICITATION Affidavit for Base Pass Memorandum for Base Pass (Site Visit Purposes) Wage Determination Drawings 13BR SPECIFICATIONS 13BR Statement of Work Solicitation number FA2823-15-R-4020 is hereby issued as a request for proposal. Potential offerors should note funds are not currently available for this project and offers should be valid through 31 Dec 2015. The Contractor shall furnish all parts, labor, tools, materials, plant, and transportation necessary to repair HVAC & Interior for Building 3017, Duke Field, Eglin AFB, FL. The contractor shall perform to the standards in the Statement of Work (SOW), drawings, and specifications, as well as all Federal, State, and local regulations. This acquisition will be a competitive total small business set aside. The North American Industry Classification System (NAICS) code for this acquisition is 238220 with a small business standard size of $15M. This acquisition will utilize the Trade-Off Source Selection Process in accordance with FAR part 15, Contracting by Negotiation and DoD Source Selection Procedures. Period of performance is expected to be 305 calendar days after award. Any resulting contract shall be firm-fixed-price. The following is the anticipated CLIN structure for the basic requirement. CLIN 0001: 1 LOT FFP Description: Repair HVAC and Interior Bldg 3017; Furnish all labor, equipment, and materials required to accomplish all of the renovation work to the Dining Facility, Bldg 3017 per the Statement of Work dated 6/25/15, Specifications and Drawings dated 4/1/15 and all other applicable documents. FTFA 13-9005. Repair HVAC & Interior, Dining Hall Bldg 3017. See Supplemental files. The Period of Performance (POP) is an estimated 305 days from the Notice to Proceed (NTP). Potential offerors should note that a site visit will be held for this requirement on 18 August 2015 at 0900. Prospective offerors must be registered in the System for Award Management (SAM). Registration can be completed by going to https://www.sam.gov/portal/public/SAM/#1 and completing the registration process. The contractor is responsible for obtaining the solicitation, amendments, and any other documentation from the FBO webpage.
