This is a sources sought only; it is not a request for quotations or offers. This procurement requirement is to determine whether any small business can provide can provide these services listed below: CLASSIFICATION CODE: Q - Medical Services; (Q504- Medical Services/Dermatology) NAICS CODE: 621 - Medical; 621111 Network-9 Contracting Office on the behalf of the Robley Rex Veterans Affairs Medical Center (VAMC), 800 Zorn Avenue, Louisville, KY 40206., has developed this Request for Information (RFI)/Sources Sought Notice to solicit market research information from industry to assist the Government with analysis of a Government requirement and provide commercial insight on performance-based solutions, best practices, possible contractual arrangements, and potential issues. This RFI is to determine the availability of Small Business (preferably Service-Disabled and/or Veteran-Owned Owned) entities within the Louisville, KY area. THIS IS A REQUEST FOR INFORMATION ONLY. It is not a Request for Proposal, a Request for Quotation, an Invitation for Bids, a Solicitation, or an indication that Network-9 Contracting Office (NCO-9) will contract for the items contained in the RFI / Sources Sought Notice. This RFI/Sources Sought Notice is part of a Government market research effort to determine the scope of industry capabilities and interest, in accordance with Federal Acquisition Regulation (FAR) Part 15.201(e), response to this RFI / Sources Sought Notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI / Sources Sought Notice are strictly voluntary and are consider as information only. The Government will not pay respondents for information provided in response to this RFI / Sources Sought Notice. Responses to this RFI / Sources Sought Notice will not be returned and respondents will not be notified of the result of the review. If a solicitation is issued, it will be announced on the Federal Supply Schedule or Federal Business Opportunity Website at a later date, and all interested parties must respond to that solicitation announcement separately from any response to this announcement. This RFI / Sources Sought Notice do not restrict the Government's acquisition on a future solicitation. Network-9 Contracting Office (NCO-9) and the Louisville VA Medical Center are conducting market research for a Contractor to furnish Dermatology Services for the Department of Veterans Affairs (VA) for the Robley Rex Veterans Affairs Medical Center (VAMC), 800 Zorn Avenue, Louisville, KY 40206. Please review the draft Statement of Work (SOW) provided below before responding to the market research questions as the Government desires that respondents offer their experience and recommendations on the following questions and issues related to this requirement. When addressing these items, respondents should consider the size and scope of the requirement described herein. STATEMENT OF WORK: A. The Contractor shall provide qualified dermatologic physicians(s) to provide inpatient (approximately 5 patients per month) and outpatient services (approximately 500 patients per month) with a minimum of four half-day clinics per week. Services will be rendered in accordance with the terms and conditions of this contract with the Robley Rex Veterans Affairs Medical Center (VAMC), 800 Zorn Avenue, Louisville, KY 40206. Contract physicians shall meet or exceed currently recognized national standards as established by The Joint Commission (TJC), and the standards of the American Hospital Association (AHA). In addition, physicians shall comply with all Veterans Health Administration's (VHA) regulations and directives. The Contractor shall provide sufficient personnel who meet all requirements herein and who can be relied on to perform in strict accordance with the contract. The Contractor shall provide physicians who are competent, qualified, and adequately trained to perform assigned duties. B. Duties: 1.The Contractor shall be available to provide both inpatient and outpatient consultation services for the diagnosis and treatment of dermatological conditions. For inpatient services, the contractor will be available seven (7) days per week, 24 hours a day. Outpatient services should be offered as needed to meet demand, but no fewer than four half-day clinics per week. The goal is to schedule all new and follow-up appointments for outpatient services within 30 days of the clinically indicated date. 2.The contractor will provide on-call information, including contact numbers, to the VA's Medical Service Office no later than three working days prior to the end of each month. This information will be published on the VA's monthly call schedule. 3. Consultation processing will be as follows: "Inpatient Consults will be the responsibility of the contractor and services are expected to be provided within 24 hours of the initiation of the consult or sooner if the condition warrants. The requesting provider is responsible for contacting the Contractor about the need for inpatient services. The consultation will be completed using a consult note in CPRS that directly links the response to the original consult and completing the associated encounter form. "Outpatient Consults will generally be received and reviewed by the VA Dermatology Nurse Practitioner within 3 working days of the consult date. In the absence of the Nurse Practitioner, the contractor will provide this coverage. Except in cases of emergency, coverage for consultation review will be scheduled in advance. The reviewer is responsible for determining the clinically indicated date. The goal is to schedule all new and follow-up appointments within 30 days of the clinically indicated date. Outpatient consults will be completed using a consult note in CPRS that directly links the response to the original consult and completing the associated encounter form. "E-Consults will generally be received and reviewed by the VA Dermatology Nurse Practitioner. In the absence of the Nurse Practitioner, the contractor will provide this coverage. Except in cases of emergency, coverage for consultation review will be scheduled in advance. E-consults will be completed by using the e-consult note and completing the associated encounter form. "Teledermatology Imaging Consults will generally be received and read by the VA Dermatology Nurse Practitioner. In the absence of the Nurse Practitioner, the contractor will provide this coverage. Except in cases of emergency, coverage will be scheduled in advance. Completion of Teledermatology Imaging Consults requires the reviewer to be designated as a "Reader" in CPRS and must complete the TMS self-study program. Once certified, the Reader will complete the Teledermatology Imaging Consults by rendering a diagnosis/recommendation using the Teledermatology consult note and closing the encounter by completing all required fields. 4.The Contractor shall direct the use of ultraviolet light (UVB) and BLU-U Light/Levulan as a treatment. 5.The Contractor shall provide subcutaneous injection of permanent or biodegradable filler material for the treatment of antiretroviral-induced facial lipoatrophy in HIV-infected patients. A. The Contractor shall provide Dermatology Services in accordance with the specifications contained herein to beneficiaries of the Department of Veterans Affairs (VA) and the Robley Rex VA Medical Center (603), 800 Zorn Avenue, Louisville, KY 40206. The contract is effective for the period October 1, 2015 through September 30, 2016, with four (4) one (1) year options to renew. B. The Authority for this procurement extends from Public Law 104-262 and Title 38 U.S.C., Section 8151-8153 and Title 42 Code of Federal Regulations (CFR) 415.170. C. All contract physicians shall be Board Certified or are eligible to sit for the board examination. Those physicians with time-limited certificates will be participating in the ABD maintenance of certification program. In addition, all contract physicians will have a full, unrestricted license to practice medicine. B. Cancellation of clinics with less than a 90 day notice shall occur only due to weather, acts of war or national disasters that are out of control of the VHA or the contract physician. All clinic reduction and cancellation requests shall be in compliance with Medical Center Memorandum No. 603-14-136-002, Clinic Set Up, Maintenance, Inactivation, and Cancellation. C. As a prerequisite to performance under the contract, the contract physician performing the services must be appropriately credentialed and privileged in this specialty area, as set forth by the VHA and VA Medical Center bylaws and policy, including board certification by an American Board and clearance by the National Practitioner Data Bank. The medical center retains the right to review the acceptability of the Contractor's employees. The Contractor shall abide by all aspects of the Medical Staff By-Laws to include medical record documentation. Any failure to meet these requirements is considered nonperformance and the basis for assessment of deduction (see QASP) and/or termination for cause, with all the attendant repercussions. D. The VA conducts Focused/Ongoing Provider Performance Evaluation (FPPE/OPPE) for each provider delivering services under this contract. The Dermatology Medical Director appointed under this contract is responsible for working with the Chief, Medical Service to develop appropriate criteria pertinent to dermatological care. Each provider's performance will be evaluated based on this defined criteria. Reviews are conducted 3 months after initial privileges are granted, then biannually and also at the time of 2-year re-credentialing. The results of these evaluations will be given to the Dermatology Medical Director to present to each provider for review and signature prior to presenting to the Hospital's Credentialing Committee for review. Signed evaluations will be returned to the Medical Service office ASAP, but not longer than 5 working days from the date given (or faxed) to the Medical Director or designee. If a provider fails to meet specific criteria, focused reviews will be conducted until compliance is reached. E. Contractor shall ensure that all VAMC mandatory training including the national training in Security Awareness and Privacy are completed during each fiscal year.
