- Appendix A Iwakuni List of Laundry M67400-15-T-0116 Iwakuni Laundry Service This is a solicitation notice/combined synopsis for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. A separate written solicitation M67400- 15 - T - 0116 has been issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-23 and Defense Acquisition Circular 91-13. DPAS Rating is DO-C9D. The North American Industrial Classification System code is 812320 and the business size standard is NTE 6.5 million. Required Item and Descriptions are as follows. ITEM NO 0001 Laundry Service (Work location: Bachelor Housing Transient Facilities Bldg. 1420, H & HS Bldg. 170, and Food Service Bldg. 1562,222,6222 ), ITEM NO 0002 Laundry Service (Work location: Medical Clinic Bldg. 111 ), ITEM NO 0003 Laundry Service (Work location: Dental Clinic Bldg. 111 ), ITEM NO 0004 Laundry Service (Work location: CLC-36 Bldg. 246 ), ITEM NO0005 Laundry Service (Work location: MAG-12 Bldg. 6250 ), ITEM NO0006 Laundry Service (Work location: MWSS-171 Bldg. 128 ), ITEM NO 0007 Laundry Service (Work location: Commissary Store Bldg. 450), and ITEM NO0008 Laundry Service (Work Location: Navy Calibration Lab Bldg. 193) The Contractor shall provide laundry services to Government facilities on located Marine Corps Air Satiation (MCAS) Iwakuni, Japan. The laundry services shall include, loading, unloading, segregating, transporting, counting and obtaining necessary documentation for items picked up or delivered. The Contractor shall pick-up, receive, account for, process and deliver all items tendered for cleaning. All items delivered and collected will take place at the Marine Corps Air Station MCAS Iwakuni, Japan. The pick-up and delivery locations are specified on Technical Exhibit 1. The Federal Acquisition Regulation (FAR) and Defense Federal Acquisition Regulation Supplement (DFARS) provisions and clauses apply to this acquisition. Responsible Offerors must be active in the System Award Management (SAM) in accordance with DFARS 252.204-7004 unless you qualify under an exemption. DUNS, TIN and CAGE Code must be written on submitted proposal. The solicitation (Request for Proposal), when it is released it will be available electronically at the following web site http://www1.fbo.gov under solicitation M67400- 15 - T-0116. All Offerors are required to submit the following as part of their Quote: (X) SF1449 (Solicitation M67400-15-T-0116) completed. (X) Offerors shall submit three separate volumes: (Volume I, II, and III) as specified in page 42~45. THE OFFERORS MUST SUBMIT ALL THE REQUIRED DOCUMENTS SPECIFIED UNDER "SUBMISSION REQUIREMENTS" IN THIS SOLICITATION TO BE CONSIDERED AS RESPONSIBLE OFFERORS. The solicitation also may be e-mailed (preferred) upon a written request to the Point of Contact listed. Upon the submission of the solicitation, it is required to attach the documents required under Submission Requirements. All offers shall be in the English language and priced in US dollar or Japanese Yen.
