- Travis Air Force Base currently has a requirement for a smoking cessation telephone and counseling line. Contracting is seeking multiple sources capable of fulfilling the requirement. Potential offerors should send a capability statement not to exceed 5 pages. 1. SCOPE OF WORK. The Quitline staff of Registered Nurses, Registered Respiratory Therapists and Smoking Cessation Counselors interacts with clients in any phase of the cessation process. They help callers work through the process of tobacco cessation. Staff spends as much time as needed with callers regarding information and education about medications as well as behavioral techniques that are helpful to quitting tobacco. The Quitline staff is extensively trained to help clients with a quitting plan that will work for them. They explain behavior modification techniques: how to change habits, deal with urges, manage stress, and much more. Nicotine products and prescription medications are described. Although products are not available from the Quitline, counselors advise clients where over-the-counter nicotine patches, gum, and lozenges are available. When the counselor feels prescription products such as the Chantix, Zyban or nicotine patches are indicated he/she will document a thorough assessment of the client’s physical and psychological state to determine the appropriateness for such a medication. If the assessment reveals no contraindications a request for a prescription, together with the written assessment will be sent via the the Contractor Webmail service to the Health Promotion Flight Commander. The Flight Commander in turn places a telephone consult to the Family Practice Tobacco Cessation Provider Liaison. The provider will then review the patient’s history and if appropriate prescribe the medication. Once the telephone consult is closed the Health Promotion Flight Commander will contact the patient and provide them with a certificate that is necessary to pick-up the medication at the 60MDG pharmacy. The Contractor Quit Line personnel follow-up with patients once a week and will also send requests for medication refills to the Health Promotion Flight Commander. This is a seamless and unique process. Furthermore the Contractor follows calls by written or emailed information designed to reinforce the verbal messages provided by the Quitline counselors. Packets are tailored to the caller’s specific needs and include information about nicotine replacement products, prescription medications, behavior modification techniques, the recovery process, withdrawal symptoms, tips for dealing with cravings, coping, stress management and much more. The the Contractor service also includes data tracking. This program is essential to the tobacco cessation program at Travis AFB because of the unique connection with medication requests and subsequent follow-up. 2. AVAILABILITY. The Contractor provides Telephone Counseling for Tobacco Cessation and requests for prescription medication if appropriate. There is a dedicated military toll free number for beneficiaries to use. Calls are answered as they are received from 0700 to 2300 CST 7days a week, if staff are all busy assisting callers, the client has the option to hold or leave a message. Messages are returned the next business day. All messages are tracked to monitor completion. 3. HOURS OF PERFORMANCE. 0700 to 2300 CST 7days a week 4. GOVERNMENT PROVIDED DUTY SCHEDULES: The processing schedule for the contract period will be annually 5. CONTINUITY OF SERVICES. Failure to provide service may subject the contractor to reimbursement of any additional cost incurred by the government. In the event counseling services cannot be performed, the Contractor must promptly notify the HAWC Flight Commander. The David Grant USAF Medical Center retains the right to seek these services elsewhere should services not provided by the Contractor. 6. ENGLISH LANGUAGE REQUIREMENT. Counseling shall be in English. 7. LICENSURE/REGISTRATION. Qualified counselors shall perform all services. The Contractor is responsible for ensuring personnel are trained counselors. 8. RECORDS. The Contractor shall provide reports regarding the use of the quit line and the quit rates at least quarterly. 9. FOR OFFICIAL USE ONLY. The contractor shall create and maintain FOUO material IAW DoD 5400-7,R, DoD Freedom of Information Act Program, AFI 33-129, Transmission of Information Via the Internet; and AFI 33-201, Communications Security (COMSEC), Table 1. Mark all documents meeting the requirements identified in DoD Regulation 5400.7/Air Force Supplement as “For Official Use Only”. Safeguard all sensitive data IAW DoD Regulation 5400.7/Air Force Supplement. When documents containing FOUO material are authorized for destruction, shred the records so that the pieces cannot be reconstructed. Degauss or overwrite magnetic tapes or other magnetic media. 10. PATIENT LISTS. Patient lists, no matter how developed, shall be treated as privileged information. Add the following to the bottom of all patient lists: “FOR OFFICIAL USE ONLY. This document contains information exempt from mandatory disclosure under the Freedom of Information Act (FOIA)., Title 5 U.S.C 552(b)(2) High and (b)(6) apply. ” Lists and/or names of patients shall not be disclosed or revealed in any way for any use outside the MTF without prior written permission by the Chief of Hospital Services. (See Attached HIPPA document) 11. PRIVACY ACT PROGRAM. The contractor shall create and maintain Privacy Act data IAW AFI 33-129, Transmission of Information Via the Internet, paragraphs 7.4, 8.2.3, 16 and 17 and supplements; AFI 33-201, Communications Security (COMSEC), Table 1; AFI 33-332, Air Force Privacy Act Program, Privacy Act systems of records notice(s), which are posted on the internet at ( http://www.defenselink.mil/privacy/notices/usaf ). The contractor shall not create or maintain a Privacy Act system of records prior to public notice. If the contractor receives a Privacy Act request, the contractor shall be responsible for searching for the records and providing those records to the government official who, as authorized official, will make the decision on releasing government records. 12. RELEASE OF MEDICAL INFORMATION. The Contractor shall release medical information obtained during the course of this contract to MTF staff involved in the care and treatment of that individual patient only. 13. CONTRACTOR FURNISHED ITEMS AND SERVICES. The Contractor shall furnish everything required to perform this contract in accordance with all of its terms.
