- Attachment 4 - RFQ Barber Shop Revised 1. The 5th Contracting Squadron, Minot AFB, ND, is seeking a vendor to operate a Non-Appropriated Fund Instrumentality (NAFI) Barber Shop concession inside Rockers Bar & Grill on Minot Air Force Base, ND 58705. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number FA4528-15-NAF01 is issued as a Request for Quotation (RFQ). This acquisition is 100 percent set aside for small businesses. The North American Industry Classification System (NAICS) code is 812111 with a size standard not to exceed $7.5 million (U.S.). This combined synopsis/solicitation is NOT to be construed as a commitment by the United States Air Force. The Government is NOT liable for any costs and/or activity associated with providing a response to this announcement. Any costs incurred as a result of a response to this announcement shall be borne by the firm and will not be charged to the government for reimbursement. 2. The General Provisions and Clauses, Articles and Special Provisions (Concessionaire Contract), Performance Work Statement (PWS), and RFQ applicable to this combined synopsis/solicitation and the resultant contract are attached. The following Federal Acquisition Regulation (FAR) and Department of Defense Supplement (DFAR) provisions and clauses also apply: 52.204-7, 52.204-13, 52.217-8, 52.217-9, 252.204-7004 Alt A, 252.232-7003. The provisions and clauses can be viewed full text through the Internet FAR site, http://farsite.hill.af.mil. 3. Description: This requirement is to operate a Barber Shop concession for a base period to begin on or about 3 August 2015 through 2 August 2016 with four (4) option years. There is space enough inside the shop for two barbers and one apprentice. All barbers must possess a valid state barber's license. 4. The Concessionaire will pay the NAFI a percentage of gross sales monthly. The government will award a concessionaire contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be the most advantageous to the government, proposed percentage and other factors considered. Award shall be based on best value to the government. Other factors considered will be the ability to demonstrate the capacity to fulfill this requirement in accordance with the attached PWS and Technical Acceptance. Technical Acceptance Criteria will be evaluated on a pass/fail basis. In order to be technically acceptable the Offeror must receive a pass rating on the following factors: •a. FACTOR 1 - PAST PERFORMANCE: This factor is met if the Offeror has been actively engaged as a contractor performing services as defined by the NAICS Code 8122111 within the last five years. Complete attached Responsibility Questionnaire (Attachment 5). •b. FACTOR 2 - QUALIFIED CANDIDATES: This factor is met when the Offeror provides a resume of a candidate possessing the qualifications stipulated in the PWS and is ready, willing, and able to perform the required duties at Minot AFB, ND according to the period of performance established in the solicitation. 5. To be eligible for contract award, vendor must be registered in the System for Award Management (SAM) and must also complete Representations and Certifications at http://www.sam.gov, per FAR 52.204-7. If the Offeror that is determined to be the best value does not become registered in the SAM database within 5 days of submitting a quote, the Contracting Officer has the option to proceed to award to the next otherwise successful registered Offeror. Processing time, which normally takes 48 hours, should be taken into consideration when registering. Offerors who are not registered should consider applying for registration immediately upon receipt of this solicitation. 5. Interested parties capable of meeting this requirement MUST submit a written quote and Responsibility Questionnaire. The quote is to include the company or individual name, complete address and phone number, tax identification number (TIN), and DUNS and CAGE code if applicable. Oral quotes will not be accepted. All responses that meet the criteria contained within will be considered. Written quotes must be received no later than 20 July 2015, 10:00 a.m. Central Time and reference the solicitation listed above. Late quotes/offers may not be considered. 6.
