- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with no additional information included in this notice. This announcement constitutes the only solicitation; quote are being requested and a written solicitation will not be issued. This solicitation number F15PS00802 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. This procurement is for SB2 Class 7 Gravel to be delivered and dumped at Jacks ¿ Bay Shop Area, approximately 12 miles east of Tichnor, AR. This requirement is being procured in accordance with FAR Part 12, Acquisition of Commercial Items and an award will be made in accordance with FAR Part 13, Simplified Acquisition Procedures. This action is being solicited as a small business set-aside and only qualified offerors may submit a quote. The NAICS Code associated with this requisition is 212312 with a small business size standard of 500 employees. Buyer intends to issue award using a fixed priced purchase order or delivery order. Bids from Sellers unable to accept purchase orders or delivery orders will not be considered for award. Offerors are cautioned that a warranted Contracting Officer is the only authorized Government Representative to make any contractual changes once an award is made. No oral statement of any person whomsoever shall, in any manner or degree modify or otherwise affect the terms of this Purchase Order. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the Purchase Order price to cover any increase in costs incurred as a result thereof. Description: This work entails furnishing all necessary management, supervision, labor, materials, licenses, certifications, and equipment to supply for delivery and tail-gate spread with a minimum of 13 truckloads delivered per day. This will be under the direct supervision of FWS Personnel. The contractor shall ensure total for all amounts supplied does not exceed the amount noted in the contract line items. (1) Line Item 0010: 370 Tons, SB2 Class 7 Gravel (2.) Line Item 0020: 370 Tons, B Stone Gravel (3) Line Item 0030: 286 Tons, Rip Rap (#125) Delivery Requirements: Material will be delivered and dumbed at Jack ¿s Bay Shop Area, approximately 12 miles, east of Tichnor, AR. Coordinate with FWS personnel about delivery quantities and exact locations. The contractor will be expected to assign more than one truck at a time to deliver on the site. There may be county road weight limit restrictions coming into this site. It is the vendor ¿s responsibility to verify site conditions prior to submitting quote and prior to all deliveries. Once delivery begins, the Contractor shall deliver at rate of not less than 10 truckloads per day consecutively with no gaps in days of non-delivery without prior approval from FWS GR. Delivery will be conducted during regular working hours, excluding federal holidays. Hours are 7:30 A.M. to 4:30 P.M; Monday through Friday. There is no restroom at the delivery site. If a restroom is needed at the delivery site, it will be provided by the contractor. Deliveries may be stopped at the discretion of the GR if rain causes the material being delivered to become saturated or causes poor road/ground conditions at the delivery site. The delivery will accompany a spot dump with amount piled and thickness to be determined by the GR at the designated location. Identified delivery location will be at designated through GR. All deliveries will occur within an active jobsite or staging area that will accommodate highway compliant vehicles, required delivery trucks, etc. Drivers shall furnish mine number stamped certified scale tickets to FWS/government personnel upon delivery of each load to the delivery site. Required commencement date for delivery is on or about on or before July 22, 2015. Expected completion date will be no later than August 31, 2015. NOTE: It is the Contractor's responsibility to visit the site prior to bidding and know existing delivery and dumping conditions. Local conditions (bridge restrictions, weather conditions, water levels, road flooding, etc.) that may affect delivery: weather conditions, road flooding and water levels. Contractor shall coordinate deliveries (including hours of delivery) with GR 48 hours in advance. Vendors must abide by all state and county weight restrictions. Comptroller General Examination of Record The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.
