- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation RFQ VA245-15-Q-0097 is issued as a request for quote (RFQ) for a Firm Fixed Price Contract. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-65. No telephone responses will be accepted. The North American Industry Classification System (NAICS) number is 541519 and the business size standard is $27.5M. This procurement is a Full and Open Competition requirement; all qualified vendors are encouraged to submit a quotation for the provision of Facial Recognition System for the Department of Veterans Affairs (VA), Martinsburg, WV. Contractor to provide: 1. Three Motorola MCC 5500 consoles. 2. Scope of Work: The contractor shall acquire and install all equipment (and software required to run such equipment) as described in this statement of work (SOW). This will include (but may not limited to) items listed under Section B. 3. Background: The purpose of this project will be to upgrade security at the medical center. 4. Type of Contract: Purchase and install. 5. Location: Installation of equipment will be at VA Medical Center, Martinsburg WV. Consoles will be located in building 318, building 500 room 1A134 and building 500 room GD108 A. GENERAL REQUIREMENTS The equipment described below will be installed in the various pre-indicated spaces. 1.Equipment will include: a.2 Motorola MCC 5500 consoles, desktop mic's, headset jacks, foot-switches, CAB, CES/CAB, also included complete spare's PAC. b.1 RS 422 Fiber MUX Conversion Equipment and Fiber Jumpers c.350 ft of Plenum Flex Innerduct "orange" d.375 ft of RS 422 Plenum cable e.Misc materials to include but not limited to grounding cable, connectors, tie wraps, and channel guard. 2. System Operation: a.The consoles will enhance the VA Police's ability to respond to emergencies on campus. 3.Service will include: a.Installation and configuration of all hardware and software to run the MCC 5500 consoles. This includes all required wiring. Contractor is responsible for supplying all equipment and materials to accomplish installation. The VA is responsible for providing adequate power within 10 ft. of the console location and proper grounding. b.All work will be completed between the hours of 7 a.m. and 4:30 p.m. Monday thru Friday except Federal holidays, unless prearranged with the COTR. 4.Testing: a.Work shall include system programing and testing. 5.Hardware Manual: a.General hardware description and specifications 6.Software Manual: a.The software manual shall describe the functions of all software, and shall include all other information necessary to enable proper loading, testing, and operation. 7.Operators Manual: a.The operators manual shall explain all procedures and instructions for operation of the system. Material to include but not limited to grounding cable, connectors, tie wraps, and channel guard Delivery shall occur during normal work hours which are 7:00 a.m. to 4:30 p.m., Monday through Friday, excluding Federal holidays (New Year's Day, Martin Luther King Day, Washington's Birthday, Memorial Day, Fourth of July, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day and Christmas Day), FAR/VAAR Clauses apply 52.252-2, Clauses Incorporated by References (Feb 1998). This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. FAR 52-204-9 Personal Identity Verification of Contractor Personnel (Sept 2007), 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items and in accordance with FAR 12.603 (b)(4), (b)(14), (b)(26), (b)(27), (b)(28), (b)(29), (b)(35), (b)(36), (b)(38), (b)(48) (c)(3), (c)(5), (c) (6). Offerors must comply with all instructions contained in provision 52.212 -1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions and clauses apply: FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. Attention: Contractors must be registered in Central Contractor Registration (CCR http://www.ccr.gov) prior to award, failure to register will result in non-responsiveness of offer. 52.216-1 Type of Contract (APR 1984) The Government contemplates award of a Firm Fixed Price contract resulting from this solicitation, 52.228-5 Insurance - Work on a Government Installation, Supplemental Insurance Requirements - In accordance with FAR 28.307-2 and the previous clause, the following minimum coverage shall apply to the contract: Worker's compensation and employers' liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so comingled with the Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in states with exclusive or monopolistic funds that do not permit worker's compensation to be written by private carriers. General Liability: $500,000 per occurrence. Automobile Liability: $200,000 per person; $500,000 per occurrence, and $20,000 per property damage.
