Description

The Department of Veterans Affairs Medical Center has a need for Community Based Health Care for Homeless Veterans (HCHV) herein called Medical Respite Rehabilitative Housing. The contractor shall furnish transitional housing services to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs Medical Center herein after referred to as the (VAMC). It is understood that the type of patients to be cared for under this contract will normally require care and services over and above the level of room and board. Homeless Veterans may have ongoing Mental illness, Depression, Schizophrenia, Post-Traumatic Stress Disorder, and/ or Substance Dependencies. This acquisition will be issued as a small business set aside. A Firm-Fixed Indefinite Deliveries/Indefinite Quantity type contract with a one (1) year base period of performance with the possibility of four (4) one (1) year option periods which will be exercised at the Government's discretion are anticipated under this solicitation. A formal Request for Proposal (RFP) will be issued in accordance with Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, and (FAR) Part 15, Contracts by negotiation on or about July 31, 2015 via FEDBIZOPPS at www.fbo.gov. It is the responsibility of potential offerors to monitor the website for downloading the solicitation and any amendments. The contractors' facility must be within a 30 mile radius of the Durham VA Medical Center. The expected performance period will be September 15, 2015 through September 14, 2016; with four option years (if exercised). The NACIS code is 624221. The size standard is $7 Million.
