Description

REQUEST FOR INFORMATION (RFI) - SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS. The Department of Veterans Affairs, Central Arkansas Veterans Healthcare System, North Little Rock, AR is conducting a MARKET RESEARCH SURVEY to obtain information regarding the availability and capability of qualified sources (Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, small disadvantaged business or large business) and their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to this sources sought announcement will be used by the Government to make appropriate acquisition method. The applicable NAICS code is 334516/size standard 500 employees DESCRIPTION OF THE REQUIREMENT: Provide instruments and proprietary reagent (reagent rental) to perform clinical laboratory testing for the detection of Procalcitonin. This will include an upcharge for all disposables and support service. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of instruments and proprietary reagent (reagent rental). Government contracts, references, and other related information; and descriptive literature. Responses should be received by July 16, 2015 no later than 2:00 PM CDT. Submissions must be made via EMAIL ONLY to: rosemary.briggs@va.gov. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER July 16, 2015 no later than 2:00 PM CDT. THIS IS NOT A SOLICITATION ANNOUNCEMENT. AT THIS TIME NO SOLICITATION EXISTS. 4) DISCLAIMER: This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract.
