- Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify ADA-compliant lodging facilities (hotels, motels, inns, etc.) that have the resources & capabilities to provide temporary lodging services for authorized Veterans of the Lebanon VA Medical Center (LVAMC). General scope of the procurement: the LVAMC intends to issue 5-year BPAs for separate, distinct temporary lodging services requirements-Social Work Service and Medical Service. 1.Social Work Service needs to lodge individual Veterans for 1 to 5 nights at a time while they receive various types of outpatient care from the LVAMC. 2.Medical Service needs to lodge up to 6 Veterans at a time for up to 9 continuous days, in the same lodging facility, about 7 times per year. These Veterans will be attending the Lebanon VAMC's Visual Impairment Services Outpatient Rehabilitation (VISOR) program training. General requirements: the lodging facility must be licensed to operate its business and be located within 10 miles (or approximately 15 minutes' drive time) of the Lebanon VA Medical Center. NCO 4 is seeking responses from lodging facilities that are interested in this procurement and consider themselves to have the resources, capabilities needed to provide the temporary lodging services. Please respond with your: 1.Lodging facility's name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), and person of contact (including telephone number & email address). 2.Lodging facility's proximity to the Lebanon VA Medical Center. 3.Facility information (e.g., amenities, handicap accessibility, van-accessible parking, restrictions, etc.). Responses must be submitted by 11:00 AM (EST) July 10, 2015.
