- W-9 FORM ACCEPTANCE OF TERMS & CONDITIONS FORM MTC VENDOR SELF-CERTIFICATION FORM DAVIS BACON WAGE SCHEDULE CONSTRUCTION BID PACKAGE WITH STATEMENT OF WORK THIS IS A SUBCONTRACTING OPPORTUNITY with Management & Training Corporation (MTC), operator of the Chicago Job Corps Center. MTC is requesting bids for the Fire S uppression System at the site location. A pre-construction meeting is scheduled for Tuesday, June 23, 2015 at 10:00am (CST). Any questions should be submitted no later than June 2 5, 2015 at 3:00pm (CST) to the point of contacts noted below. All bids and associated documents are due no later than June 2 9, 2015 at 3:00pm (CST). Services provided under this solicitation are under NAICS Code #237110 (Water and Sewer line Related Structures Construction). Information regarding this NAICS code is located at http://www.census.gov/cgi-bin/sssd/naics/naicsrch. MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final.
