- BLANK W-9 FORM MTC SELF-CERTIFICATION FORM MTC TERMS AND CONDITIONS SCA WAGE DETERMINATION REQUEST FOR QUOTE This is a subcontracting opportunity with Management and Training Corporation (MTC), operator of the Flint Hills Job Corps Center (FHJCC). Please accept this as your invitation to attend our pre-bid meeting for HVAC Control System Evaluation at FHJCC located at 4620 Eureka Dr., Manhattan, KS 66503.　 The meeting will be conducted on July 7, 2015, @ 9:00 AM (CST) with bids due on Friday, August 7, 2015 @3:00 PM (CST). The attached Request For Quote contains the requirements of the project. The Service Contract Act Wage Schedule, W-9, MTC terms and conditions,　 and MTC Vendor Self-Certification forms are provided for your convenience. All forms, references must be returned with our Bid Sheet in order to be deemed responsive. MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final. MTC may elect to award contract on a per deficiency basis.
