- Solicitation Number: VA-241-15-Q-0588 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA-241-15-Q-0588 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-74, May 30, 2014. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as a small business set-aside. The North American Industry Classification System (NAICS) code is 611310, and the size standard is $27.5 million dollars. (v) This requirement consists of three (3) line items: Part 1: Decontamination Training Suite - Minimum 8-hour comprehensive Best Practices for the Protection of Healthcare-Based First Receivers, Complete Course curriculum which combines the successful completion of the following: - One-hour instructor-led facilitation of online course as or equal to Yale New Haven Health Center for Emergency Preparedness & Disaster Response's (YNHHS-CEPDR) Awareness level course EM120 for up to 50 learners. - Three-hour instructor-led facilitation of online course as or equal to YNHHS-CEPDR Best Practices for the Protection of Hospital-Based First Receivers, Operations Level EM220 for up to fifty learners. - Four-hour Practical skills workshop for up to fifty learners. - One-hour Train-the-Trainer Program for up to six identified learners. Part 2: Decontamination Full Scale Exercise - Full scale exercise at West Haven CT VA Medical Center - Three planning meetings at West Haven CT VA Medical Center. The planning for this exercise will also include an initial, mid-term, and final planning meeting. A Master Scenario Events List (MSEL) Conference will be held if warranted by the exercise design. The contractor's exercise teams will work collaboratively with the identified West Haven VA Emergency Management Staff to form an exercise planning team. The team will form the purpose, scope, objectives, and scenario for the evacuation exercise. All exercise materials will be developed. - Exercise Lead and Lead Evaluator - All exercise controllers, evaluators and planning team members will participate in a Controller/Evaluator briefing prior to the exercise. A Controller/Evaluator Handbook will be developed by contractor and will include all materials contained in the Player Handbook along with Exercise Evaluation Guidelines and the evaluation plan. - Three evaluators in addition to exercise director for day of exercise - On the day of exercise, contractor will provide the two staff who conducted the exercise planning and design as well as two additional controller I evaluators. - Development of all training and exercise materials specific to West Haven VAMC location - After Action Report Part 3: Evacuation Full Scale Exercise - Full scale exercise at West Haven CT VA Medical Center - Three planning meetings at West Haven CT VA Medical Center. (vi) Additional Requirements - This training directly corresponds to and addresses VA Emergency Preparedness Act of 2002, TJC Standard EM 02.02.05, and OSHA 29 CFR 1910.120 HAZWOPER Compliance CPL. - Training must begin as soon as possible but no later than July 15, 2015. Period of Performance is not to exceed one year. - Provide access to annual online refresher training in future years either through the TRAIN program at no charge or via other acceptable LMS. - All exercise controllers, evaluators, and planning team members will participate in a Controller/Evaluator briefing prior to the exercises. A Controller/Evaluator Handbook will be developed by contractor and will include all materials contained in a Player Handbook along with Exercise Evaluation Guidelines and the evaluation plan. On the day of exercise, contractor will provide the two staff that conducted the exercise planning and design as well as two additional decontamination expert evaluators. - Immediately following the exercises, a hotwash with exercise participants will be conducted and participants will be asked to complete a debrief survey. Exercise evaluation documents and the participant debrief survey will be analyzed by contractor for inclusion in the After Action Reports (AAR). One draft (within 30 days of exercise) and one final AAR (within 60 days of exercise) will be provided to West Haven VA. (vii) Delivery FOB Destination to West Haven VAMC, 950 Campbell Avenue, West Haven, CT 06516 (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i)Technical capability of the item offered to meet the Government requirement (ii)Price (iii)Past Performance (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- Offeror's are to include a COMPLETED COPY OF PROVISION 52.212-3 WITH THE SUBMITTED QUOTE, unless offeror has a current ORCA certification.
