Description

Statement of Work, Terms and Conditions It is the intent of the Phoenix Area Indian Health Service (IHS) to acquire inpatient meal preparation and delivery services for the Colorado River Service Unit/Parker Indian Health Center, located in Parker, AZ. Therapeutic and regular meals shall be provided for patients of the inpatient department on a daily basis, three (3) meals per day; quantity per day is dependent on actual patient load on any given day. The term of the contract shall be for a period of one base year, including four one-year option periods. All costs associated with the performance of this contract shall be responsibility of the contractor. The RFQ is available immediately and may be obtained by written request only to the above address or email address: tochukwu.aniche@ihs.gov. Only written requests shall be honored. Proposals will be accepted through June 26, 2015; 1PM, MST.
