Description

Contractor to return this packet along with response to evaluation criteria Complete Solicitation THIS COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a complete package is attached for offerors to return. This notice constitutes as Solicitation AG-04T0-S-15-0040 and is being issued as a Request for Quotation. This notice and any incorporated provisions include those in effect through Federal Acquisition Circular 2005-82. Quotes are being solicited from Small Business Concerns only. The NAICS code associated with this acquisition is 324121 and the small business size standard is 500 employees. Description: Supply Hot Mix asphalt to be picked up by Umpqua National Forest Roads. Must be able to deliver Oregon DOT Level 3 half inch (1/2") dense hot mix asphalt. Supplier must not be more than 2 hours one way commute time for a loaded agency dump truck to deliver the Hot Mix to our road system jobsites. PERIOD OF PERFORMANCE: The anticipated contract period is July 15, 2015 through September 30, 2015. The following provisions and clauses are hereby incorporated into this RFQ in full text in the attachment "AG-04T0-S-15-0040Solicitation.pdf: and FAR 52.212-4, Contract Terms and Conditions-Commercial Items (May 2015), FAR 52.212-2, Evaluation- Commercial Items (Oct 2014), FAR 52.212-3, Offerors Representations and Certifications-Commercial Items (Mar 2015) and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes of Executive Orders-Commercial Items (May 2015) QUOTE DUE DATE: Quotes shall be received by July 1, 2015, 4:00 pm PST. Quotes may be sent by fax to 541-750-7236 or mailed to Siuslaw National Forest, 3320 SW Jefferson Way, Corvallis, OR 97331. Quotes cannot be accepted by email.
