Description
Wage Determination information Updated RFQ Packet. Please fill out specified areas and return by end date of 06/15/2015. Updated SOW. All areas must be met to qualify for award. Amendment to solicitation HSBP1015Q0095 to provide updated SOW and solicitation information as well as updated program name. (1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (2) The solicitation number is HSBP1015Q0095 and is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-80. The associated North American Industrial Classification System (NAICS) code for this procurement is 423450 with a small business size standard of 100 employees. (3) This procurement shall be a small business set-aside. All qualified, responsible sources may submit an offer that shall be considered by the agency. Offerors are advised that they are responsible for obtaining amendments. The resulting contract shall be awarded to the offeror who provides the lowest-priced technically acceptable quote in compliance with the Statement of Work (SOW). (4) The Department of Homeland Security (DHS), Customs and Border Protection (CBP) intends to award a contract to establish a Buffalo Field Office (BFO) AED/CPR Response Program in accordance with the specifications listed in the attached SOW. (5) The purpose of this procurement action is to establish an agreement with a responsible contractor to conduct a site assessment and review of current AED placement in CBP facilities throughout the Buffalo Field Office, review current program and response protocols, and to develop the BFO AED/CPR Response Program to meet or exceed Federal guidelines, including training of volunteer BFO AED responders. Additionally, the selected contractor shall be capable of handling the majority of the administrative functions (including equipment monitoring, upkeep and replacement) necessary to ensure program continuity and minimize the amount of effort required by BFO staff to coordinate and administer the BFO AED/CPR Response Program. Please see attached SOW for further details of requirement. (6) It is required that all contractors submitting a quote review the attached STATEMENT OF WORK to review all requirements. All contractors submitting a quote shall be required to meet the requirements found in the STATEMENT OF WORK (7) The vendor selected for award MUST register in System for Award Management (SAM) at https://www.sam.gov/portal/SAM/#1 and receive payment via Electronic Funds Transfer (EFT) as a condition of the contract. Registration in SAM requires a valid Dun & Bradstreet (DUNS) number. No award can be made to a company not registered in SAM. Eastern Time (Indianapolis). No phone calls will be accepted. Late submissions will not be accepted.
