Description

Solicitation Package This combined synopsis/solicitation provides moving services for the relocation of the San Carlos Indian Health Service Hospital from San Carlos, AZ to a new medical center located in Peridot, AZ. The Department of Health and Human Service, The Phoenix Area Indian Health Service (IHS) is soliciting quotes to relocate the San Carlos Indian Health Service Hospital from San Carlos, AZ to a new medical center located in Peridot, AZ. IAW 52.219-6, Notice of Total Small Business Set-aside (Nov 2011), this procurement is 100% set-aside for small businesses. Offers received from firms other than small businesses will not be considered. NAICS: 484210 SIZE STANDARD: $25.5 Million The mandatory Period of Performance is July 7-10, 2015. A Pre-bid site walk is scheduled for June 11, 2015 at 1:00 p.m. AZ Time, at the San Carlos Indian Health Service Hospital, 223 Cibecue Circle, San Carlos, AZ. Participants at the site visit will meet in the main lobby of the hospital. Offerors are urged and expected to attend this meeting and inspect the site.
