- THIS IS NOT A SOLICITATION NOTICE This is a market survey for information and planning purposes only. This Request for Information (RFI) serves to improve the understanding of Government requirements and industry capabilities regarding Home Oxygen Services and allows potential offerors to judge whether or not the Government's requirements can be satisfied within offerors' capabilities. Funding is not available at this time. North American Industry Classification System (NAICS) code: 621610. BACKGROUND The James A. Haley Veteran's Hospital (JAHVH), 13000 Bruce B. Downs Blvd., Tampa, FL 33612, has a requirement for home oxygen delivery services to provide medical-grade home oxygen to the residence of approximately 1200 veterans. These deliveries shall be provided within a 106 mile radius of the JAHVH, not to exceed Umatilla, FL. Deliveries of refills shall be made Monday - Friday (excluding specified holidays) between the hours of 7:30 AM and 3:00 PM. Weekend deliveries are required when needed for discharges. If for any reason the contractor is unable to deliver at the agreed upon day or time, the contractor will provide a 12 -hour notice to the COR, find an alternate source and initiate its backup plan. The Contractor shall have an emergency response plan that provides for emergency deliveries of oxygen within 6 hours after notification, 24 hours a day, 7 days a week. Tanks Es, M9 - BA, AND M6 -AA shall be filled to maximum functional capacity at each refilling procedure unless otherwise specified in the facility requirements. All contractor-owned equipment, such as monitoring devices, shall be installed in accordance with NFPA 50: (CGMP) Regulations. The contractor shall comply with all OSHA standards and applicable safety requirements, including proper signage and use of personal protective equipment. The Contractor will coordinate with the (HRCP) Operations Manager on all home deliveries and pickups of oxygen tanks, liquid rentals, H tanks and liquid oxygen systems. The James A. Haley Veterans' Hospital requires the following home oxygen services. Estimated quantities are listed below: a.Liquid Rentals = 30 to 60 per month b.Liquid Pounds = 9,000 per month c.H Tank Rentals = 500 to 700 per month d.Average Cylinder Refills = 15,000 per month e.Delivery and set-up (H Tanks and Liquid) The Contractor will maintain 400 Aluminum E Oxygen Cylinders for VA use during Hurricane Season (June 1- November 30). Rental of a Liquid Oxygen System must be no more than 6 months old. Specific requirements include: a.Two (2) 100 pounds Companion Liquid Oxygen Reservoirs will be provided for each Veteran receiving Liquid Oxygen. b.Examples are Companion, or comparable 1- Secures 2 portable liquid systems with canellas, carrying bag or portable transport cart 2- H Tanks, regulator, and stand (VA will supply regulator and stand). The required response time of the unscheduled work requests shall not exceed two hours. The contractor shall possess all permits and licenses required by Federal, State and local laws, regulations and ordinances demonstrating that they are in fact qualified by training and experience to perform the required work. The Contractor shall have 24 hour/7 day emergency service available for delivery or repair of liquid systems. The Contractor shall ensure the tanks are in compliance with applicable Federal and State Regulations. Annual tank testing/inspections, i.e., hydrostatic tests or leak pressure test requirements by NFPA, shall be coordinated with the COR. Invoices shall be submitted separately on a monthly basis. The Contractor shall provide all equipment and materials necessary to perform the requirements of the contract. Government owned and rented oxygen tanks shall be labeled according to any applicable FDA, DOT, OSHA, or other requirements. The projected period of performance shall begin approximately 1 DEC. 2015. The applicable NAICS for this service is 621610. The Government is interested in Small Businesses, preferably Service Disable Veteran Owned Small Businesses (SDVOSB), to provide these services. Interested and qualified vendors are encouraged to: 1)Send a summary of their company's capabilities, and company history/information, license to operate in West Central Florida (Tampa, St. Petersburg - Clearwater Metropolitan Statistical Area) and any information you deem beneficial to this RFI. 2)Describe capability to meet requirement of Non-Manufacturing Rule (FAR 19.102 (4)) for equipment (i.e., nasal cannula, tubing, cylinder w/oxygen regulator, cylinder handcart, etc.). 3)Include your company's point of contact, address, e-mail address, telephone, DUNS, Socio-economic identification (WOSB, SDVOSB, VOSB, etc.).
