Description

NCA 8 SAO EAST, VA Caribbean Healthcare System, San Juan, Puerto Rico intends to negotiate a commercial (FAR Part 12 and 15) This action is for the procurement of services to provide Phlebotomy Services for Vieques, Puerto Rico. Applicable North American Industry Classification System (NAICS) code number 621511 and Size Standard are $30M. This is a Request for Quote (RFQ) Award will be made to a responsible offeror on the basis of the lowest price for services requested. QUESTIONS: shall be submitted in writing via e-mail attention to the Contract Specialist at hector.lopez-burgos@va.gov. No personal visit, telephone inquiries or faxed will be accepted. All responses will be answered and incorporated in to a written amendment to the solicitation, for the benefit of all interested offeror's. Solicitation No. VA248-15-Q-1418 electronic Request for Information package will be available to the public as Request for Quote at FBO web site by or after JUN 02, 2015. Offerors shall go to FBO web site to download subject RFQ package, including any related attachments and/or amendments afterJUN 02, 2015. In case the RFQ due date is extended, all interested offeror's shall be responsible to monitor the changes posted at FBO web site, prior the RFQ due date. Each offeror shall acknowledge receipt of each amendment, submitting each amendment to hector.lopez-burgos@va.gov by the RFQ due date. All interested offeror's shall be registered in the System for Award Management (SAM) database. Contractors are responsible for the accuracy of information added to SAM. To be registered, you must go to https://www.sam.gov/sam/. Current contract information may be re-quested and disclosed under Freedom of Information Act (FOIA), in accordance with 38 Code of Federal Regulations (CFR) 1.553(b).
