OKC OK VA Medical Center Oklahoma City, OK 73104 BRIEF SCOPE OF WORK (This is for information only) Background The contractor shall provide all required labor, material and equipment, including travel cost to perform preventive maintenance, repair, calibrate and certify American Medical Systems (AMS) HPS Green Light Laser equipment, Model# ????, Seral# HPS2761. Project Scope Contractor shall provide all required labor, material and equipment, including travel cost to perform preventive maintenance, repair, calibrate and certify American Medical Systems (AMS) HPS Green Light Laser equipment, Model# ????, Seral# HPS2761. Contractor shall repair equipment back to OEM specifications. Using only OEM manufactured or approved new parts. No refurbished, used or re-stocked parts, and no gray market parts. Technicians shall have adequate experience, training, and applicable licensing, to perform required preventative maintenance. Contractor shall provide equipment specific vendor trained individuals to service the equipment. Contractor shall provide all documentation/certificates showing completion of equipment specific training from the vendor by the individual conducting the repair. Location of performance Oklahoma City Veterans Administration Medical Center (OKC VAMC) 921 NE 13th St. Oklahoma City, OK 73104 IMPORTANTR NOTE: This RFI (Request for Information) is for information only; no award will be made off of this RFI. Please communicate via e-mail to Lance.Davis2@va.gov by COB June 3, 2015 as to your company's ability to perform service per this brief Scope of Work.
