- STATEMENT OF WORK (SOW) TITLE: Testing of Airborne Infection (Negative Pressure) in Isolation Rooms. A.General Description: Monthly testing of Airborne Infection (Negative Pressure) Isolation Rooms is required to ensure HVAC equipment is functioning properly to maintain negative pressure in such designated rooms. Contractor shall furnish all labor, supervision, materials, and quantitative measurement equipment necessary to accomplish complete monthly testing of airborne infection (negative pressure) isolation rooms at New York Harbor Healthcare System. Location: New York Harbor Healthcare System Brooklyn Campus 800 Poly Place Brooklyn, NY 11209 Locations of Airborne Infection (Negative Pressure) Isolation Rooms: 13 West12 West*12 East *11 West 11 East (ICU)8 West6 West4 West ER 13-304W12-307W12-224E11-302W11-225E8-302W6-312W (Dialysis)4-301 (GI)G-512 13-305W12-308W12-225E 11-303W11-227E8-303WG-513 **11-228E Notes: *Unit not being used for airborne infection at this time and not required to be tested at this time. ** 11-228E is a designated positive pressure room and must be tested on a monthly basis for positive pressure with equipment which can provide a quantitative measurement. B.Work Hours: 1.Normal Work Hours: The service schedule will be developed between the contractor and Contractor's Technical Representative (COTR) prior to any service being performed not to exceed a 30 day period. 2.National Holidays: The ten holidays observed by the Federal Government are: New Year's Day, Martin Luther King's Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, Christmas Day, and any other day specifically declared by the President of the United States to be a national holiday. C.Specifications: 1.Contractor must perform in accordance with JCAHO, CDC, ASHRAE, & DHHS standards and recommendations. 2.The monthly testing schedule will be closely adhered to prevent inconsistent and unnecessary call-ins. 3.The contractor shall assume full responsibility for compliance with all federal, State, city, and local laws, rules, and regulations governing testing of airborne infection (negative pressure) isolation rooms as well as specific VA directives governing testing of airborne infection (negative pressure) isolation rooms. When such rooms are occupied by patients on airborne precautions and/or contact precautions the contractor is required to follow New York Harbor Healthcare System's implemented Occupational Safety and Health Administration (OSHA) Respiratory Protection Program under OSHA 29 CFR 1910.134 and/or any CDC guidelines. 4.Contractor shall provide and bring quantitative methodology testing equipment. This equipment shall be certified and inspected on an annual basis. This equipment shall also be cleaned and disinfected at the contractor's facility in compliance with the equipment manufacturer's guidelines and as warranted. 5.Airborne infection (negative pressure) isolation rooms listed shall be tested monthly. These rooms shall be tested with use of equipment which provides a quantitative measurement. 6.The positive pressure isolation room listed shall be tested monthly. This room shall be tested with use of equipment which provides a quantitative measurement. 7.New York Harbor Healthcare System's Brooklyn Campus reserves the right to delete or add the number of airborne infection (negative pressure) isolation rooms being tested monthly as deemed necessary (i.e. Influx of Patients/Cohort of Patients due to Influenza). D.Call Backs: 1.Emergent/Non-Emergent Calls: Contractor shall report within a mutually agreed upon time with COTR for non-scheduled services without additional charge to the Government. E.Quality Assurance: The Government will investigate complaints of New York Harbor Healthcare System. All information received will be provided either verbally and/or in writing to the contractor for appropriate action. F.Performance Objective: Contractor provides monthly testing of negative pressure isolation rooms and one positive pressure room designated in ICU. G.Performance Threshold: No more than one failure per contract period. H.Required Reporting: 1.The contractor is responsible for supplying, completing, and submitting all reports required or requested by Federal, State, or local ordinances, which pertain to any duties contained in the contract. 2.The contractor is responsible for supplying, completing, and submitting all reports required to New York Harbor Healthcare System's Brooklyn Campus designee on a monthly basis within five (5) days in order for any deficiencies to be corrected by the facility to prevent disruption in patient care. 3.The contractor is responsible for supplying and posting signage with use of a removable mechanism at the time of monthly testing on the entry doorway and/or doorway of airborne infection (negative pressure) isolation rooms and/or the positive pressure isolation room alerting patients, visitors, and staff that the room is not operating as designed. Contractor signs shall read as follows: "Do Not Use for Respiratory Isolation." I.Contractor Parking: 1.It is the responsibility of the contractor's personnel to park in the appropriate designated parking areas. 2.New York Harbor Healthcare System does not validate or make reimbursement for parking violations of the contractor's personnel under any circumstance. J.Interference to Normal Function: 1.Contractor may be required to interrupt their work at any time so as not to interfere with the normal functioning of the facility, including utility services, fire protections systems, and passage of facility patients, personnel, equipment, and carts. 2.In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the Government within the 30-day time period. 3.Contractor personnel shall inform the COTR or the designee of the need to gain access to secured areas. If access is required to secure areas, prearranged scheduling will be made with COTR or designee. Note: All contractor employees and subcontractors under this contract or order are required to complete the VA's on-line "VA Privacy and Information Security Awareness and Rules of Behavior Training" - TMS Code "10176". Contractors must provide signed certifications of completion to the COTR during each year of the contract. This requirement is in addition to any other training that may be required of the contractor and subcontractor(s)." K.Proposal Evaluation Processes: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1.Technical Capabilities: a.UNDERSTANDING OF THE REQUIREMENT - The Government will evaluate the offeror's understanding of the requirement in accordance with the Statement of Work (SOW). The offeror shall submit a proposal that demonstrates its understanding of work described in the SOW. b.CORPORATE EXPERIENCE - The offeror shall submit a summary of experience of a similar nature, scope, complexity, difficulty, and size to that which will be supplied under the prospective contract contemplated by this solicitation. c.KEY PERSONNEL - The offeror's offer shall reflect the experience and qualifications of all contractor personnel. 2.Past Performance: The offer shall provide a list of Past Performance references. This list shall be submitted with the proposal when responding to the solicitation. Offerors are advised that the Government may use past performance information obtained from centralized past performance databases and sources other than those identified by the offeror and the information obtained may be used for both the responsibility determination and the best value decision. Contracts listed may include those entered into with Government agencies or private sectors. 3.Cost/Price: The Government will evaluate offers for award purposes by adding the total of all CLIN prices to ensure that pricing is not unbalanced. Price will be evaluated for completeness and reasonableness using FAR 12.209 and FAR 15.4. Price will become significantly more important as technical ratings approach equality. L.Basis for Award: The Government will award a single Contract from this solicitation to the responsible offeror whose offer conforming to the solicitation is most advantageous to the Government based on the best overall proposal that is determined to be the most beneficial to the Government, with appropriate consideration given to the three evaluation factors: Technical, Past Performance and Price. Technical and past performance, when combined, are approximately equal to cost or price.
